目的：探讨外伤性基底节区血肿（ TBGH）的发病机制、临床特点、诊断及治疗。方法回顾性分析我院45例外伤性基底节区血肿患者的临床资料。结果45例病例中，手术治疗15例，保守治疗30例。伤后6个月GOS评分：良好GOS 5分24例，中度残疾，生活能简单自理GOS 4分7例。重度致残GOS 3分12例，持续植物生存GOS 2分，0例，死亡GOS 1分2例。结论外伤性基底节区血肿发病率低而致残率较高，患者的年龄、血肿大小、颅内压、伴发其他颅内损伤等因素均可影响预后，早期积极治疗是改善预后的关键。
목적：탐토외상성기저절구혈종（ TBGH）적발병궤제、림상특점、진단급치료。방법회고성분석아원45예외상성기저절구혈종환자적림상자료。결과45례병례중，수술치료15례，보수치료30례。상후6개월GOS평분：량호GOS 5분24례，중도잔질，생활능간단자리GOS 4분7례。중도치잔GOS 3분12례，지속식물생존GOS 2분，0례，사망GOS 1분2례。결론외상성기저절구혈종발병솔저이치잔솔교고，환자적년령、혈종대소、로내압、반발기타로내손상등인소균가영향예후，조기적겁치료시개선예후적관건。
Objective To discuss the pathogenesis, clinical characteristic, diagnosis and treatment of the traumatic basal gangliahematomas (TBGH).Methods The clinical data of 45 patients with TBGH were analyzed retrospectively.Results Of 45 patients with TBGH, 15 were taken operation, 30 were taken the conservative treatment.GOS of 6 months after injury showed 24 cases were in good prognosis,7 in moderate disability,12 in severe disability,none was in vegetative state and 2 died.Conclusion The incidence of TBGH is low and the morbidity rates is high,the key to improve the prognosis is due to the early diagnosis,a positive and appropriate treatment.
