目的：总结肛瘘术后复发原因并提出对策。方法将我院于2009年1月至2013年12月47例多次肛瘘手术患者进行分析。结果术中发现22例患者内口未正确处理,17例患者术中支管未充分切开引流不畅,4例患者出现假愈合,2例3次手术后不愈合,病理检查诊断为结核,抗痨治疗痊愈,2例患者4次手术后证实为克隆恩病,未完全愈合。结论肛瘘术后复发的原因主要是术前诊断不明确,内口处理不当,支管处理不彻底,切口引流不畅,切口假愈合。

Objective To explore the main causes,prevention and treatment measures of postoperative re-currence for anal fistula.Methods The data on clinical recurrence of the 47 cases who were analyzed admit-ted from Jan 2009 to Dec 2013.Results The internal opening were not handled correctly in 22 cases,and the branch fistula tract were disregarded in 17 cases.False healing were devolped in 4 cases.Two cases of intestinal tuberculosis were healed by anti-tuberculosis treatment.Two cases of Crohn's disease were not completely healed.Conclusion Preoperative diagnosis unclearly,internal opening handled uncorrectly, branch fistula tract disregarded,uncorrect thread-drawing method and false healing were the main causes postoperative recurrence of anal fistula.
