目的：：探讨社区健康管理模式在老年高血压患者中的实施效果。方法：选择我社区近期收治的100例高血压老年患者作为本次研究的实验对象。将100例患者随机分成观察组和对照组，各50例。观察组给予健康管理模式干预，对照组不实施干预。比较两组血压干预前后变化、高血压知识掌握情况、自我保健意识比率、高血压并发症，评价健康管理模式的效果。结果：观察组干预后患者高血压知识掌握程度、自我保健意识均比干预前高，血压和并发症情况均低于干预前，前后比较P＜0.05，对照组干预前后无变化，P＞0.05。结论：社区健康管理模式在老年高血压患者应用中取得了良好的效果，普及了老年人高血压健康知识，提高了老年患者自我保健意识，减少了不良并发症，提高了老年人生存质量。
목적：：탐토사구건강관리모식재노년고혈압환자중적실시효과。방법：선택아사구근기수치적100례고혈압노년환자작위본차연구적실험대상。장100례환자수궤분성관찰조화대조조，각50례。관찰조급여건강관리모식간예，대조조불실시간예。비교량조혈압간예전후변화、고혈압지식장악정황、자아보건의식비솔、고혈압병발증，평개건강관리모식적효과。결과：관찰조간예후환자고혈압지식장악정도、자아보건의식균비간예전고，혈압화병발증정황균저우간예전，전후비교P＜0.05，대조조간예전후무변화，P＞0.05。결론：사구건강관리모식재노년고혈압환자응용중취득료량호적효과，보급료노년인고혈압건강지식，제고료노년환자자아보건의식，감소료불량병발증，제고료노년인생존질량。
[ABSTRACT]Objective: To investigate the effect of community health management model for hypertension in the elderly. Methods:100 cases of elderly patients with hypertension in our community treated as the experimental object in this research. 100 patients were randomly divided into observation group and control group, 50 cases in each. The observation group was given health management intervention, the control group without intervention. Change, compared two groups before and after the intervention of blood pressure of hypertension knowledge, self health awareness rate, complications of hypertension, evaluate the effect of health management model. Results:In the observation group, the prognosis of patients with dry hypertension knowledge mastering the self health care consciousness degree, have high blood pressure than before, and the complications were lower than those before intervention, compares the P<0.05, no changes in the control group before and after the intervention, P>0.05.Conclusions:Community health management model has achieved good effect in the application of elderly patients with hypertension in the elderly hypertensive, popularization of health knowledge, enhance the consciousness of self health care of elderly patients, reduce the complication, improve the survival quality of elderly people.
