目的:观察广谱caspase抑制剂(z-VAD-fmk)对大鼠烫伤后肾脏组织的保护作用。方法72只雄性Wistar大鼠随机分为对照组、致伤组和给药组,每组24只,各组设2、8、24、48h共4个相应时段,每时段6只,给药组致伤后即刻经尾静脉注射 z-VAD-fmk 3mg/kg,以后1.5mg/kg每12h经腹腔注射1次,致伤组用同样方法注射与给药组等体积的生理水,对照组不做处理,致伤组和给药组烫伤后用Parkland公式补液,并根据给药量相应减少补液量,分别于上述各时段处死大鼠并留取肾脏组织,行HE染色观察肾组织的病理改变,TUNEL法检测肾脏细胞的凋亡,荧光比色法检测Caspase-3活性。结果与致伤组相比,给药组病理切片显示肾脏组织损伤有所减轻;肾脏细胞凋亡指数(AI)在烫伤8、24及48h后明显降低(P<0.01);caspase-3活性在伤后2、8、24及48h均明显降低(P<0.01)。结论早期应用z-VAD-fmk治疗可以有效地减少肾脏细胞的凋亡,进而减轻肾脏的损伤,起到保护肾脏的作用。

목적:관찰엄보caspase억제제(z-VAD-fmk)대대서탕상후신장조직적보호작용。방법72지웅성Wistar대서수궤분위대조조、치상조화급약조,매조24지,각조설2、8、24、48h공4개상응시단,매시단6지,급약조치상후즉각경미정맥주사 z-VAD-fmk 3mg/kg,이후1.5mg/kg매12h경복강주사1차,치상조용동양방법주사여급약조등체적적생리수,대조조불주처리,치상조화급약조탕상후용Parkland공식보액,병근거급약량상응감소보액량,분별우상술각시단처사대서병류취신장조직,행HE염색관찰신조직적병리개변,TUNEL법검측신장세포적조망,형광비색법검측Caspase-3활성。결과여치상조상비,급약조병리절편현시신장조직손상유소감경;신장세포조망지수(AI)재탕상8、24급48h후명현강저(P<0.01);caspase-3활성재상후2、8、24급48h균명현강저(P<0.01)。결론조기응용z-VAD-fmk치료가이유효지감소신장세포적조망,진이감경신장적손상,기도보호신장적작용。
Objective To observe protective effect of z-VAD-fmk on kidney in rats after-scald. Methods Seventy-two male Wistar rats were randomly assigned into control group, injury group and treatment group, 24 rats in each group. Four time points of 2h, 8h, 24h and 48h were set in each group separately. For the treatment group, z-VAD-fmk was administrated immediately at the dosage of 3mg/kg by caudal vein injection and 1.5mg/kg per 12h by intraperitoneal injection.The injury group was only given normal saline with the same method and volume as treatment group ,and the control group received nothing. Sodi-um lactate ringer's injection was administered in the injury group and treatment group after scald according to the formula of Parkland and the sodium lactate ringer's injection reduced correspondingly. The rats were killed and the kidneys were immedi-ately removed respectively at each time point mentioned above. Renal morphology dyed with HE was observed with microscope. Detection of apoptotic renal cells was carried out using TUNEL and activity of caspase-3 was measured with colorimetric chro-matometry. Results Compared with the injury group,the damage of rat renal cells were lessened in treatment group,the apop-tosis index (AI) of renal cells significantly decreased at 8h,24h and 48h (P<0.01) the change of caspase-3 activity at 2h, 8h, 24h and 48h significantly decreased after scald (P<0.01). Conclusion The renal cells apoptosis participated in renal injury after-scald. The early intervention of z-VAD-fmk could effectively reduce renal cells apoptosis and relieve kidney damage.
