目的:评价急性ST段抬高性心肌梗死STEMI患者行急诊经皮冠状动脉介入( PCI)术时经血栓抽吸后冠脉应用替罗非班对梗死相关血管血流及临床预后的影响。方法将114例患者随机分为替罗非班组与对照组,比较两组TIMI血流、TMPG分级、心功能的改变、出血事件以及住院期间MACE事件率。结果替罗非班组术后TIMI血流及TMPG分级均优于对照组,LVEF明显高于对照组(P<0.05),住院期间MACE低于对照组(P<0.05),两组出血事件及LVEDD差异无统计学意义(P>0.05)。结论血栓抽吸后冠脉内应用替罗非班是处理冠状动脉内血栓安全、有效的方法,可减少慢血流或无血流的发生,改善心肌灌注及近期预后。

목적:평개급성ST단태고성심기경사STEMI환자행급진경피관상동맥개입( PCI)술시경혈전추흡후관맥응용체라비반대경사상관혈관혈류급림상예후적영향。방법장114례환자수궤분위체라비반조여대조조,비교량조TIMI혈류、TMPG분급、심공능적개변、출혈사건이급주원기간MACE사건솔。결과체라비반조술후TIMI혈류급TMPG분급균우우대조조,LVEF명현고우대조조(P<0.05),주원기간MACE저우대조조(P<0.05),량조출혈사건급LVEDD차이무통계학의의(P>0.05)。결론혈전추흡후관맥내응용체라비반시처리관상동맥내혈전안전、유효적방법,가감소만혈류혹무혈류적발생,개선심기관주급근기예후。
Objective To evaluate the effects of intracoronary tirofiban on blood flow and clinical outcomes of STEMI patients after thrombus aspiration during primary PCI. Methods A total of 114 cases were randomly divided into the tirofiban group and the control group. The TIMI flow, TMPG classification,changes in heart function, bleeding events and MACE events rate during hospitalization were compared between the two groups. Results The TIMI flow and TMPG grading of the tirofiban group were superior to those of the control group, LVEF was also significantly higher than that of the control group (P < 0. 05), MACE during hospital was lower than that of the control group (P< 0. 05), and bleeding events and LVEDD were similar between the two groups (P > 0. 05). Conclusion Use of intracoronary tirofiban af-ter thrombus aspiration is safe and effective in preventing the slow-flow or no-flow phenomenon, and it can improve myocardial perfusion and recent prognosis.
