目的:探讨构建以公共卫生服务为导向的社区健康服务绩效管理模式,以促进社区公共卫生服务的发展。方法将公共卫生服务量作为绩效考核的主要指标,并根据公共卫生服务项目的难易程度与重点推进项目相结合设定不同的标化系数,绩效工资总额的40%以标化工作量作为分配依据。以公共卫生服务为导向的绩效管理模式实施前后,对设置绩效考核指标的9项公共卫生工作10个工作指标及同期门诊量进行同一口径的数量统计,并进行对比。采用匿名电子问卷调查的方式来了解社康中心员工对新型绩效管理方案的真实意见。结果241名员工中有188人提交了问卷,其中有153人(81.4%)认为一级方案能促进社区公共卫生工作的发展,125人(66.5%)支持一级方案实施,133人(71.1%)认为通过绩效管理体现了自己的劳动价值及贡献。2013年1—12月公共卫生服务量比2012年1—12月大幅增加,2013年高血压、糖尿病、精神病患者的管理人数分别增加了107.7%、110.8%、246.8%,新建健康档案增加93.1%,儿童保健增加76.1%,老年人保健增加40.1%,预防接种增加25.3%,孕妇保健增加9.8%,门诊量增加12.6%。结论以公共卫生服务为导向的绩效管理模式可以有效促进社区公共卫生服务的发展。

Objective To discuss a performance management model which is able to improve the community public health service. Methods The key performance indicator was the volume of the public health service,and a standardized coeffi-cient was set up based on the complexity level of public health service programs and major promoted projects. Forty percentage of the performance-related pay was distributed according to the standardized workload. Before the application,quantitative statistics was made and unified concerning 9 public health service indexes,10 workload indexes and the outpatient volume at the same time. The truthful opinions of the employees in the community public health centers were obtained by anonymous electronic ques-tionnaire investigation. Results 188 out of 241 employees returned their questionnaires. 153 ( 81. 4%) thought first class per-formance-related pay distribution plan could promote the development of the community health public services,125(66. 5%) were in favor of its application and 133(71. 1%)thought it embodied their labor value and contribution. The public health serv-ice increased considerably in volume from January to December of 2013 compared to that in the same period of 2012. The numbers of hypertension,diabetes mellitus and psychopaths under managed rose by 107. 7%,110. 8% and 246. 8% respectively,the newly-set health records by 93. 1%,child care by 76. 1%,senile medical care by 40. 1%,preventive vaccination by 25. 3%, pregnant women health care by 9. 8% and the outpatient volume by 12. 6%. Conclusion The public health service oriented per-formance management model can improve the development of community public health service effectively.
