通过建立以县人民医院为龙头，县-社区医疗联动的转诊模式，依托“双向转诊中心、影像会诊中心、临检中心”建设，搭建县和社区医院的预约转诊信息化网络平台，完善转诊流程和管理制度，建立便捷顺畅的转诊管理，推进双向转诊。本文重点介绍县-社区医疗联动的主要操作模式、规范转诊过程管理的具体措施及实施效果。

Through the county hospital-oriented,county-community medical linkage referral patterns,relying on the construction of" two-way referral center,image consultation center,clinical examination center",to build up the referral information network platform of the county and community hospital appointment,to improve the referral process and management system,to establish convenient and smooth referral management and to promote two -way referral system. The main operation mode,specific measures of referral process management and implementation effect of the county-community medical linkage are discussed in the article.
