目的对家庭医生助手在社区高血压患者管理中的效果进行评估。方法通过对本社区100例重点组高血压患者进行2年的随访管理，运用护理指导和健康教育，对患者的服药的依从性进行管理同时对患者的血压进行监测，运用优良、尚可、不良对患者每个管理年的血压控制情况进行评估。结果本社区2012、2013年高血压患者的服药依从为98%、血压控制情况呈上增趋势。结论家庭医生助手有助于社区高血压患者的随访管理，提高的患者规律服药的依从性，促进其改善生活方式和饮食习惯，帮助其控制血压。

Objective The ef ect assessment of the family doctor assistant about manage in hypertension patients in community.Methods Based on 100 cases of focus group of hypertension patients in community for 2 years of fol ow-up management.Applying nursing instruction and health education,to manage the patient's medication adherence to the patient's blood pressure monitoring,at the same time using the excel ent,fair,poor to assess each management in patients with blood pressure control.Results This community in 2012 and 2013 the medication compliance of hypertension patients (98%),blood pressure control with increasing trend.Conclusion The family physician assistant help fol ow-up management of hypertension patients in community,improve the regularity of patients medication compliance,improve their lifestyle and eating habits,help to control blood pressure.
