目的 采用Meta分析的研究方法分析血清中基质金属蛋白酶-3(MMP-3)与骨关节炎(OA)的关系及临床诊断意义.方法 检索国内外有关基质金属蛋白酶-3与骨关节炎的对照实验研究,按照纳入标准纳入相关的文献.对纳入文献进行数据整理、使用ReviewManager4.2软件进行统计学分析和分析各研究间异质性.结果 共纳入国内外文献7篇,各研究间存在较高异质性(χ 2=180.74,P<0.00001,F=96.7%),合并标准化均数差(standardized mean difference SMD)为2.88[95%CI为(1.61~4.16)]、整体效果检验(test for roverall effect)的Z值为4.44,P<0.00001.结论 骨关节炎患者血中MMP-3含量比正常人显著升高.但鉴于MMP-3与OA相关研究的高质量文献较少,证据仍不充分,尚需要严格设计的大样本的实验研究来进一步证实其关系.

Objective To evaluate the relationship of serum MMP-3 between osteoarthritis ( OA )disease. Methods Relative studies were included from Pubmed , CNKI and VIP database according to included and extracted standards. Heterogeneity and standardized mean difference were analyzed by the ReviewManager4.2 software. Results 7 studies were included in the analysis. There was high significant heterogeneity among the 7 studies ( χ 2=180.74, P < 0.000 01, I2=96.7% ). Standardized mean difference ( SMD ) is 2.88 ( 95%CI 1.61 ~ 4.16 ), the Z value is 4.44 ( P < 0.000 01 ). Conclusions Serum MMP-3 in OA patients was significantly higher than that in healthy people. However, due to the potential problem of selection bias, performance bias and publication bias, the current evidence was not strong enough to eonelud that increasing MMP-3 concentration can diagnose OA. It was suggested that large scale randomized studies should be performed.
