目的探讨核苷酸类药物治疗基因型HBV患者的护理方法,以提高他们的生活质量和延长生存期.方法采用前瞻性队列研究的临床研究方法,将2010年6月至2012年6月收治的慢性乙型病毒性肝炎患者260例,随机分为拉米夫定组100例、阿德福韦酯组100例和空白对照组60例,前两组使用焦磷酸测序法在抗病毒治疗前后检测HBV基因型和耐药基因,随访追踪72周,对照组不进行随访及相关检测.根据所选的药物,由专业人员负责提供整体护理及全程药物督导,进行有针对性的健康教育,做好随访及用药指导.结果通过医生或护士对患者有针对性的指导和健康教育,观察组(拉米夫定组、阿德福韦酯组)对服药时间、药物的作用掌握情况较对照组明显提高,为100％；而未进行健康教育的对照组擅自停药或自行换药的比例达15％.结论核苷酸类药物治疗乙型病毒性肝炎可致一定人群耐药,治疗期间做好患者药物方面的护理指导及健康教育工作对改善患者的遵医行为至关重要.

Objective To explore the nursing methods for nucleotide drugs therapy in patients with hepatitis B virus (HBV) genotypes,and improve their quality of life and prolong survival time.Methods By using prospective cohort study of clinical research methods,from June 2010 to June 2012,260 cases of chronic viral hepatitis b,were randomly divided into lamivudine and adefovir group (all 100 cases) and control group (60 cases),the former two groups used pyrophosphoric acid sequencing method in antiviral treatment before and after all testing HBV genotype and drug resistance genes,follow-up tracking for 72 weeks,no follow-up and related inspection in control group.According to the selected drugs by professional personnel responsible for providing holistic nursing care and the drug supervision,all patients were given targeted health education,with the follow-up and medication guide.Results Through the doctors or the nurses' targeted guidance and health education,the medication time and the mastering level to medicines effect in observation group improved more obviously than control group,for 100％;but not for health education of controls,withdrawal or without authorization by the proportion of dressing up to 15％.Conclusion Nucleotide drugs for HBV can cause a population,drug resistance during treatment with drugs to the nursing instruction and health education are very important to improve the patients' behavior of following doctors'instruction.
