目的研究BiPAP呼吸机联合呼吸兴奋剂治疗AECOPD并肺性脑病的应用效果.方法将66例AECOPD并肺性脑病患者随机均分成A、B、C三组.A组给予尼可刹米静脉滴注;B组给予经口鼻面罩双水平气道正压通气(BiPAP)呼吸机正压通气,C组在BiPAP呼吸机正压通气的同时静滴尼可刹米.结果治疗结束后,A组患者在血气分析、呼吸频率、心率等指标方面无显著改善(P＞0.05);B、C组在接受治疗5d后,各项指标均有显著改善(P＜0.05),且C组患者各项指标改善较B组更明显,差异有统计学意义(P＜0.05).结论 BiPAP呼吸机联合呼吸兴奋剂尼可刹米治疗AECOPD并肺性脑病可以有效改善患者的病情,提高患者的生存质量.

Objective To explore the efficacy of bi-level positive pressure (BiPAP) ventilation combined with respiratory stimulant in the treatment of pulmonary encephalopathy in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods 66 AECOPD patients with pulmonary eneephalopathy were randomly divided into the group A,group B and the group C.The group A received intravenous drip of nikethamide; group B received BiPAP ventilation; group C received BiPAP ventilation and intravenous drip of nikethamide.Results Compared with the group A and the group B,the group C had improved significantly in the blood gas analysis index,respiratory rate,heart rate and other indicators (P ＜ 0.05).Conclusion The use of nikethamide combined with BiPAP ventilation in the treatment of AECOPD patients with pulmonary encephalopathy can significantly improve the patients' conditions and their quality of life.
