目的 探讨非肥胖的阻塞性睡眠呼吸暂停综合征(obstructive sleep apnea syndrome,OSAS)血清载脂蛋白B(apoB)/载脂蛋白A(apoA-I)比值的变化及可能的影响因素.方法 检测118例非肥胖OSAS患者及118例性别、年龄、体质量指数匹配健康对照组的血清apoA-I、apoB、甘油三酯、总胆固醇、高密度脂蛋白-胆固醇、低密度脂蛋白-胆固醇、肌酐、葡萄糖等生化指标,计算apoB/apoA-I比值,进行统计学处理及分析.结果 非肥胖OSAS患者的apoB/apoA-I比值(0.82±0.256)、apoB[(0.95±0.229) g/L]、甘油三酯[(1.53±0.912) mmol/L]较对照组[(0.70±0.200)、(0.88±0.202) g/L、(1.21±0.597) mmol/L]升高(t值分别为-3.921,-2.493,-3.242,P<0.05);高密度脂蛋白-胆固醇[(1.09±0.301) mmol/L]低于对照组[(1.18±0.274) mmol/L],两组之间差异有统计学意义(t =2.609,P<0.05).在控制了BMI的偏相关分析显示,apoB/apoA-I与睡眠呼吸暂停低通气指数呈正相关(偏相关系数=0.498,P<0.05).结论 非肥胖OSAS患者存在血脂代谢异常,apoB/apoA-I比值与OSAS严重程度相关.

목적 탐토비비반적조새성수면호흡잠정종합정(obstructive sleep apnea syndrome,OSAS)혈청재지단백B(apoB)/재지단백A(apoA-I)비치적변화급가능적영향인소.방법 검측118례비비반OSAS환자급118례성별、년령、체질량지수필배건강대조조적혈청apoA-I、apoB、감유삼지、총담고순、고밀도지단백-담고순、저밀도지단백-담고순、기항、포도당등생화지표,계산apoB/apoA-I비치,진행통계학처리급분석.결과 비비반OSAS환자적apoB/apoA-I비치(0.82±0.256)、apoB[(0.95±0.229) g/L]、감유삼지[(1.53±0.912) mmol/L]교대조조[(0.70±0.200)、(0.88±0.202) g/L、(1.21±0.597) mmol/L]승고(t치분별위-3.921,-2.493,-3.242,P<0.05);고밀도지단백-담고순[(1.09±0.301) mmol/L]저우대조조[(1.18±0.274) mmol/L],량조지간차이유통계학의의(t =2.609,P<0.05).재공제료BMI적편상관분석현시,apoB/apoA-I여수면호흡잠정저통기지수정정상관(편상관계수=0.498,P<0.05).결론 비비반OSAS환자존재혈지대사이상,apoB/apoA-I비치여OSAS엄중정도상관.
Objective The aim of this study was to investigate the apoB/apoA-I ratio in nonobese patients with obstructive sleep apnea syndrome (OSAS).Methods One hundred and eighteen nonobese patients with OSAS and 118 age,gender and BMI matched healthy individuals without signs and symptoms of apnea-hypopnea received biochemistry analysis.Results In nonobese OSAS group,the apoB/apoA-I ratio (0.82 ± 0.256),apoB[(0.95 ± 0.229) g/L],TG [(1.53 ± 0.912) mmol/L] were higher than control group[(0.70±0.200),(0.88±0.202) g/L,(1.21±0.597) mmol/L](t =-3.921,-2.493,-3.242,P <0.05),and HDL-C[(1.09 ± 0.301) mmol/L] were lower compared with control group [(1.18±0.274) mmol/L](t =2.609,P <0.05).After controlling for BMI,the apoB/apoA-I ratio was correlated with AHI (partial correlation coefficient =0.498,P < 0.05).Conclusions OSAS can induce lipid metabolism disorder in nonobese patients and the apoB/apoA-I ratio was correlated with severity of OSAS.
