目的探讨血清N-糖基化修饰蛋白-高尔基体蛋白73(GP73)的表达水平与HBV感染后肝脏疾病转归的关系.方法收集绍兴市第六人民医院2010年10月至2013年5月期间354例肝病患者的血清样本,其中原发性肝癌81例、重型肝炎123例、慢性乙型肝炎57例和乙型肝炎肝硬化93例,用ELISA法检测患者血清GP73,并同时选取30例健康体检者血清GP73进行对照.结果原发性肝癌组、重型肝炎组、乙型肝炎肝硬化组、慢性乙型肝炎组和健康对照组血清GP73含量中位数分别为345.2、301.7、262.0、123.5及58.6 ng/mL,组间两两比较差异均有统计学意义(u=891.0、958.0、1029.5、419.0,P＜0.05或＜0.01).原发性肝癌组病毒载量≥3 lg拷贝/mL组(52例)血清GP73中位数为328.3 ng/mL,显著高于病毒载量＜3 lg拷贝/mL组(29例)的284.3 ng/mL,且差异有统计学意义(u=533.5,P＜0.05).相关性分析显示,血清GP73与胆碱酯酶、白蛋白之间呈负相关(r=-0.57、-0.59,P＜0.01).结论 HBV感染者病情越重,病毒载量越高,GP73表达水平也越高,血清胆碱酯酶和白蛋白表达水平与GP73表达呈负相关.

Objective To investigate the relationship between serum N-glycosylated Golgi protein 73 (GP73) expression and outcome of liver disease after HBV infection.Methods A total of 354 serum sample were collected from patients with liver diseases (81 primary hepatic cancer,123 severe hepatitis,57 chronic hepatitis B,and 93 hepatitis B associated liver cirrhosis) in the Six People's Hospital of Shaoxing during October 2010 and May 2013.ELISA was used to detect serurm GP73.Thirty healthy people were enrolled as controls.Results The median serum levels of GP73 in primary hepatic cancer group,severe hepatitis group,liver cirrhosis group,chronic hepatitis B group and healthy controls were 345.2,301.7,262.0,123.5,and 58.6 ng/mL,respectively,with significant differences between every two groups (u=891.0,958.0,1029.5 and 419.0,P＜0.05 or ＜0.01).The median serum level of GP73 in primary hepatic cancer patients with viral load ≥ 3 lg copies/mL (52 cases) was higher than that of primary cancer patients with viral load ＜3 lg copies/mL (29 cases),with a significant difference (328.3 ng/mL vs 284.3 ng/mL,u=533.5,P＜0.01).Serum GP73 expression was negatively related to cholinesterase and albumin (r=-0.57 and-0.59,P＜0.05).Conclusions Serum GP73 expression increases as the liver disease becomes severe and viral load rises.It shows a negative correlation of GP73 with cholinesterase and albumin.
