目的 观察脓毒症时不同时间点不同部位B1细胞计数和分泌IgM能力的变化,以初步探索B1细胞在脓毒症中的作用. 方法 收集脓毒症患者和健康对照者外周血样本,流式细胞术检测两组人群外周血B细胞和B1细胞比例.取18只C57BL/6小鼠采用随机数字表法随机分为假手术组(Sham组)和盲肠结扎穿孔(cecal ligation and puncture,CLP)组,根据CLP的次数又进一步分为CLP-1组和CLP-2组,即分别穿刺1次和2次,每组6只.取外周血、脾脏和腹腔灌洗液,流式细胞术检测B细胞和B1细胞计数和百分比的变化,腹腔B1细胞分泌膜IgM的变化. 结果 ①流式细胞术检测结果显示脓毒症患者外周血B细胞和B1细胞百分比为(9±4)%和(18±8)%,低于健康对照者(16±6)%和(36±5)%(P<0.05);②小鼠CLP-1组术后24 h,外周血中B细胞和B1细胞计数为(6.00±4.20)×102/ml和(3.86±2.88)×102/ml,显著低于Sham组(3.29±0.39)×103/ml和(2.04±0.28)×103/ml(P<0.05);③CLP-1组小鼠术后24 h,脾脏中B细胞和B1细胞计数为(9.13±3.24)×105/ml和(2.08±0.71)×105/ml,显著低于Sham组(6.71±0.71)× 106/ml和(1.29±0.88)×106/ml(P<0.05);④CLP-1组小鼠术后24 h和48 h腹腔中的B细胞百分比较Sham组降低(P<0.05),而B1细胞在6h和24 h降低(P<0.05),48 h无变化;⑤严重脓毒症时(即CLP-2组)小鼠B1细胞分泌IgM百分比为(61±7)%,较Sham组减少(84±6)%(P<0.05). 结论 脓毒症患者和小鼠B1细胞的数量和功能均发生负向变化,提示B1细胞可能参与了脓毒症诱导的免疫功能障碍.

Objective To investigate the change of expression on B1 cells in different sites at several times in sepsis and to explore the role of B1 cells in sepsis.Methods Blood samples were collected from sepsis patients and nomal persons,C57BL/6 mice were randomly divided into Sham group and cecal ligation and puncture (CLP) group,accroding to the times of puncture,CLP group was further divided to CLP-1 group and CLP-2 group,as puncture once and puncture twice respectively.Blood sample,spleen,and peritoneal lavage fluids were collected.The number and the percentage of B cells and B1 cells as well as the change of IgM which was secreted by B1 cells in abdominal cavity were measured by flow cytometry.Results ① The results of flow cytometry revealed that the percentage of B cells and B1 cells in the blood of patients in sepsis is (9±4)% and (18±8)%,relatively lower than healthy volunteers (16±6)% vs (36±5)%(P<0.05).② In mice,the number of B cells and B1 cells in blood of CLP-1 group is (6.00±4.20)×102/ml and (3.86±2.88)×102/ml,significant lower than Sham group (3.29±0.39)×103/ml vs (2.04±0.28)×103/ml (P<0.05).③ The number of B cells and B1 cells in spleen of CLP-1 group is (9.13±3.24)×l05/ml and (2.08±0.71)×l05/ml,significant lower than Sham group (6.71±0.71)×106/ml vs (1.29±0.88)×106/ml(P<0.05).④ As to Sham group,the percentage of B cells in peritoneal cavity of CLP-1 group after CLP 24 h and 48 h in mice is decreased (P<0.05).However,only at the timc of 6 h and 24 h,the percentage of B1 cells is declined(P<0.05).5 The percentage of secretion of IgM in B1 cells is (61±7)% when mice experienced severe sepsis in CLP-2 group,which is lower than Sham group(84±6)%(P<0.05).Conclusions Both the number and the function of B1 cells have negative changcs,it's suggested that B1 cells may involved in immune dysfunction of sepsis.
