目的 明确黑龙江省耐多药结核分枝杆菌耐药相关基因katG、rpoB和inhA的突变特征.方法 收集黑龙江省肺结核患者痰培养的1 50株耐多药以及35株对一线药物敏感的结核分枝杆菌;应用PCR法扩增菌株的katG、rpoB及inhA基因,并进行序列测定;应用DNAMAN 6.0.3.99软件分析耐药基因核苷酸及氨基酸突变情况.结果 该地区耐多药结核分枝杆菌katG基因优势突变位点为S315,占全部katG基因突变菌株的90.3%;inhA启动子区突变以C-15T为主,占85.4%;rpoB基因突变位点以S531为主,占72.5%;其次是S526突变,占19.7%;katG联合inhA突变的菌株有6株(4.0%),其中5株(3.3%)有3种基因突变.上述耐药基因优势突变类型的比例与国内其他地区有一定的差异.结论 该地区耐多药结核分枝杆菌耐药相关基因优势突变类型与国内其他地区无明显差异,但优势突变位点的构成比与国内其他地区不同.

Objective To investigate the mutation characteristics of drug resistance genes of multidrug-resistant M.tuberculosis in Heilongjiang province.Methods 150 muhidmg-resistant and 35 drug-sensitive M.tuberculosis isolates were obtained from clinical specimens of pulmonary tuberculosis patients in Heilongjiang province.The katG,rpoB,and inhA genes were amplified by PCR and sequenced.The mutation of genes and the changes of amino acids were analyzed by DNAMAN 6.0.3.99.Results Among the muhidrug-resistant M.tuberculosis isolates in the studied area,mutation at S315,accounting for 90.3% of all mutation types in katG gene,are the dominant.C-15T in inhA promoter region accounted for 85.4% of all mutation types.Of the muhidrug-resistant M.tuberculosis isolates containing rpoB gene mutations,72.5% have S531 mutation; 19.7% have S526 mutation.Six isolates (4.0 %) have combined mutations in katG and inhA genes ; Five isolates (3.3 %) have combined mutations in three genes.The proportions of dominant mutations of the drug resistant genes in this area are varied comparing with those in other areas of China.Conclusion There are the similar mutation types but different proportions of dominant mutations of the genes associated with drug-resistant in the multidrugresistant M.tuberculosis isolates in Heilongjiang Province comparing with other areas of China.
