目的探讨血液灌流对急性中毒患者凝血功能的影响.方法检测32例急性中毒患者第1次行血液灌流前、后的凝血指标和血小板数值及观察临床出血情况.结果血液灌流治疗1h后,血小板数值与治疗前比较差异有统计学意义(P＜0.05),但凝血指标无明显变化,除了2h后D-二聚体有显著升高(P＜0.05)；6例患者出现明显的出血,15例患者因凝血而被迫终止治疗.结论血液灌流影响血小板的数值,不影响其他凝血过程,充分的抗凝治疗和正确的预充是防止凝血的关键.
목적 탐토혈액관류대급성중독환자응혈공능적영향.방법 검측32례급성중독환자제1차행혈액관류전、후적응혈지표화혈소판수치급관찰림상출혈정황.결과 혈액관류치료1h후,혈소판수치여치료전비교차이유통계학의의(P＜0.05),단응혈지표무명현변화,제료2h후D-이취체유현저승고(P＜0.05)；6례환자출현명현적출혈,15례환자인응혈이피박종지치료.결론 혈액관류영향혈소판적수치,불영향기타응혈과정,충분적항응치료화정학적예충시방지응혈적관건.
Objective To explore the effects of hemoperfusion on coagulation function with acute poisoning patients.Methods Coagulation index,amount of platelet and clinical hemorrhage events were detected and observed in 32 cases of patients with acute poisoning before and after hemoperfusion(HP).Results The amounts of platlet were significantly reduced after one hour HP treatment(P ＜ 0.05),coagulation index was not changed except that D-D dimers were significantly elevated(P ＜ 0.05) ; 6 cases presented with significant bleeding,15 cases were forced to terminate treatment due to coagulation.Conclusions HP affects the amounts of platlet,but do not affect other coagulation process.Fully anticoagulation and correct priming is the key to prevent blood coagulation.
