目的 探讨甲状腺乳头状癌(papillary thyroid carcinoma,PTC)组织中DKK3基因mRNA表达、其启动子区甲基化状态及其与PTC的发生、发展的相关性.方法 运用甲基化特异性PCR(MSP)检测49例PTC及其相对应癌旁组织(NCE)中DKK3基因启动子甲基化状态,采用逆转录聚合酶链反应(RT-PCR)检测该基因的mRNA表达.结果 在NCE中,无一例出现DKK3基因启动子甲基化,而在PTC中38.8%出现甲基化;NCE和PTC中,DKK3基因mRNA表达的阳性率分别为100%和44.9%,2组差异均有统计学意义(P <0.01);PTC中DKK3基因启动子甲基化及mRNA表达与患者的病理学分级、TNM分期、淋巴结转移有相关性(P<0.05).DKK3基因启动子甲基化与其mRNA表达有明显的相关性(P<0.01).结论 DKK3基因启动子甲基化是基因失活的重要机制之一,在PTC的发生、发展中起重要作用.

목적 탐토갑상선유두상암(papillary thyroid carcinoma,PTC)조직중DKK3기인mRNA표체、기계동자구갑기화상태급기여PTC적발생、발전적상관성.방법 운용갑기화특이성PCR(MSP)검측49례PTC급기상대응암방조직(NCE)중DKK3기인계동자갑기화상태,채용역전록취합매련반응(RT-PCR)검측해기인적mRNA표체.결과 재NCE중,무일례출현DKK3기인계동자갑기화,이재PTC중38.8%출현갑기화;NCE화PTC중,DKK3기인mRNA표체적양성솔분별위100%화44.9%,2조차이균유통계학의의(P <0.01);PTC중DKK3기인계동자갑기화급mRNA표체여환자적병이학분급、TNM분기、림파결전이유상관성(P<0.05).DKK3기인계동자갑기화여기mRNA표체유명현적상관성(P<0.01).결론 DKK3기인계동자갑기화시기인실활적중요궤제지일,재PTC적발생、발전중기중요작용.
Objective To study the relationship between status of methylation of DKK3 gene promoter and expression of its mRNA in papillary thyroid carcinoma (PTC).Methods Methylation-specific PCR (MSP) and RTPCR were applied respectively to examine the methylation of DKK3 gene promoter and its mRNA expression in 49 samples of PTC and their matched adjacent non-cancerous epithelium(NCE).Results No case had methylation of DKK3 gene promoter for NCE.In PTC,the rate of methylation DKK3 gene promoter was 38.8% (19/49),significantly higher than that in NCE(P <0.01).The positive expression rate of DKK3 gene mRNA was 100% in NCE and 44.9% (22/49) in PTC.The difference had statistical significance (P < 0.01).In PTC,the hypermethylation of DKK3 gene and the expression of DKK3 mRNA had correlation with pathological grading,TNM stage,and lymph node metastasis (P <0.05).There was distinct correlation between methylation of DKK3 gene promoter and expression of its mRNA (P <0.01).Conclusion Methylation of the DKK3 promoter might be one of the mechanisms resulting in transcription silence,and might play an important role in carcinogenesis and progression of PTC.
