目的探讨蓝光治疗对高胆红素血症(HB)新生儿排尿方式的影响.方法选取出生后3～14d确诊为轻中度HB的新生儿26例.观察HB新生儿同日内蓝光治疗期(12:00-16:00)和间歇期(无蓝光照射)(08:00-12:00)各4h的自由排尿情况,HB新生儿按常规持续匀速静脉液体治疗.每次排尿前后尿垫重量的差值记作每次排尿量,残余尿量通过超声测得.分别记录排尿时间、排尿次数、排尿量、残余尿量,比较蓝光治疗期和间歇期相关参数的异同.结果共进行了112例次排尿观察,蓝光治疗期45次,蓝光治疗间歇期67次.每次排尿量2.1 ～ 103.3 mL,平均(19.73±13.16) mL.患儿多不能完全排空膀胱,常有残余尿.蓝光治疗期残余尿测得率为75.6％(34/45例),残余尿(11.73±10.81) mL;蓝光治疗间歇期残余尿测得率为77.6％(52/67例),残余尿(13.65 ±9.72) mL.蓝光治疗期HB新生儿排尿次数、每次排尿量和排尿总量分别为(1.73±1.00)次、(16.26 ±9.34) mL和(30.83 ±24.45) mL,显著低于蓝光治疗间歇期[(2.58±0.70)次、(20.84±9.05) mL和(54.32 ±30.20) mL],差异均有统计学意义(P均＜0.05);而蓝光治疗期和间歇期HB新生儿的残余尿量差异无统计学意义(P＞0.05).结论蓝光治疗对HB新生儿排尿方式有显著影响.

Objective To explore the effect of phototherapy on urinating pattern in newborns with hyperbilirubinemia(HB).Methods A total of 26 newborns aged 3-14 days after birth diagnosed with mild to moderate HB were included in this study.Eight hours of free urinating pattern of the newborns was observed per day,including 4 hours (12:00-16:00) with phototherapy and 4 hours(08:00-12:00) without phototherapy.The liquid was input into all newborns through vein uniformly and continuously by standard protocol in both periods.The urinating time,urinating volume and post-urinating residual(PVR) volume were recorded.The diaper weight difference before and after urinating was defined as urinating volume and the PVR volume was determined by ultrasound.The urinating frequency,total urinating volume,average urinating volume and average PVR volume in phototherapy period and non-phototherapy period were calculated.Data were analyzed by paired t-test.Results A total of 112 times of urinating were recorded,45 times in phototherapy period and 67 times in non-phototherapy period.The urinating volume per time ranged from 2.1 to 103.3 mL[(19.73 ± 13.16) mL] in both phototherapy period and nom-phototherapy period.Most newborns could not void the bladder completely and the voiding was often accompanied by PVR volume.In phototherapy period,the PVR volume measured rate was 75.6％ (34/45 cases) and the PVR volume was (11.73 ± 10.81) mL.In addition,the PVR volume measured rate was 77.6％ (52/67 cases) and the average PVR volume was(13.65 ± 9.72) mL in nonphototherapy period.Compared with the non-phototherapy period,the urinating frequency,average urinating volume and total urinating volume in 4 hours were significantly lower than those in the phototherapy [(1.73 ±1.00) times vs (2.58 ± 0.70) times,(16.26 ±9.34) mL vs (20.84 ± 9.05) ml,and (30.83 ± 24.45) mL vs (54.32 ± 30.20)mL],respectively and there were significant differences,respectively(all P ＜ 0.05).However,there was no difference of average PVR volume between the two periods(P ＞0.05).Conclusion There was a significant change of urinating pattern in HB newborns with phototherapy.
