目的 分析骨外科手术后切口感染的情况,为术后患者伤口感染预防及防治提供依据.方法 2009年1月至2011年12月,根据本院骨科住院手术患者伤口情况,留取骨科手术后切口感染的伤口分泌物并送微生物室进行检验,对其中病原菌的分布和耐药性进行相关因素分析.结果 病原微生物的感染与手术切口的分类有关,其中Ⅰ类切口的感染率低,而Ⅱ类和Ⅲ类切口的感染率高[ 5.9%( 14/237)比39.2%( 93/237)、54.8%(130/237),均P<0.05].术后患者切口感染的阳性菌以金黄色葡萄球菌为主,阴性菌以大肠埃希菌为主.结论 骨外科手术患者切口感染原因多样,应注意提高患者免疫力.感染时应根据感染病原菌进行抗生素的个性化治疗.

Objective To analyze the wound infection after orthopedic surgery and to provide the evidence to prevent and treat the wound infection for post-operative patients.Methods Between January 2009 and December 2011,the secretion of wound infection after orthopedic surgery was collected and sent to moicrobiology lab for detection according to the wound conditions of patients in Department of orthopedics of our hospital.Associated factors analysis of the distribution and drug-resistance of pathogens was performed.Results The infection of pathogenic microorganisms was associated with the classification of surgical wound.The infection rate of class Ⅰ wound was low,while the infection rates of classes Ⅱ and Ⅲ wounds were relatively higher [5.9 (14/237) vs 39.2% (93/237),54.8% (130/237),both P<0.05].In the wound infection after surgery,the gram-positive bacteria was mainly Staphylococcus aureus and the gram-negative bacteria mainly E.coli.Conclusions There are various factors to cause wound infection after orthopedic surgery.Thus,great attentions should be paid to improve the immunity of patients.Furthermore,the individual treatment of antibiotics should be performed according to pathogenic bacteria.
