目的 研究乙型肝炎病毒相关性肾炎(HBV-GN)患者的尿液蛋白质谱特点,筛选差异表达的蛋白质.方法 用丙酮沉淀法提取HBV-GN患者(n=7)、原发性膜性肾病伴慢性乙型肝炎患者(n=5)、原发性膜性肾病患者(n=7)、慢性乙型肝炎无肾脏损害患者(n=7)及健康受试者(n=6)尿液标本中的蛋白质,酶解后行高效液相色谱-电喷雾离子阱串联质谱(HPLC-ESI-IT-MS/MS)检测.得到的肽序列结果用MASCOT软件行数据库检索,用查得的蛋白序列号在Uniprot数据库中进行检索,查询相关的蛋白质的信息.5组数据用手工方法进行组间对比,分析HBV-GN患者尿液中差异表达的蛋白质.结果 HBV-GN患者的尿液中检测出4种具有差异性表达的蛋白质,分别是α1抗胰凝乳蛋白酶、Igγ2链C区、前列腺素D2合酶、未命名蛋白(C9JAM6).结论 α1抗胰凝乳蛋白酶、前列腺素D2合酶有可能成为HBV-GN的生物标志物.

Objective To analyze the characteristics of urine proteomics in patients with hepatitis B virus associated-glomerulonephritis (HBV-GN) for screening the differentially expressed proteins.Methods The urinary proteins were precipitated with acetone in 7 cases with HBV-GN,5 with primary membranous nephropathy complicated with chronic hepatitis B,7 with primary membranous nephropathy,7 with chronic hepatitis B without renal damage and 6 healthy subjects.This entailed enzyme cleavage followed by highperformance liquid chromatography-electrospray ionization-ion trap tandem mass spectroscopy (HPLC-ESI-IT-MS/MS) for deriving the peptide sequences that were subsequently retrieved from the database by using MASCOT software.This was followed by protein sequence searching based on the Uniprot database.The among-group difference was compared by manual calculation to analyze the differentially expressed proteins in the urine of HBVGN patients.Results A total of 4 differentially expressed proteins,namely alpha-1-antichymotrypsin,Ig gamma2 chain C region,prostaglandin D2 synthase and C9JAM6 (an unnamed protein),were identified in the urine of HBV-GN patients.Conclusion Alpha-1-antichymotrypsin and prostaglandin D2 synthase may be the potential biomarkers of HBV-GN.
