目的 探讨利多卡因对肠缺血-再灌注(ischemia/reperfusion,I/R)损伤幼鼠肠组织表达肿瘤坏死因子α(TNF-α)和细胞间粘附分子1(ICAM-1)的影响.方法 将48只4周龄SD大鼠随机分成三组,每组16只:假手术组(Sham组),仅暴露腹腔脏器,不作任何干预;缺血-再灌注组(I/R组)和利多卡因干预组(Lid组),阻断肠系膜上动脉造成缺血60 min,I/R组再灌注前经股静脉注射生理盐水0.5ml,Lid组再灌注前经股静脉注射利多卡因2 mg/kg.于再灌注后30、60、90、120min每组动物各处死4只取小肠组织进行免疫组织化学染色,检测肠组织TNF-α、ICAM-1的表达,结果进行组间单因素方差分析.结果 正常肠组织中TNF-α和ICAM-1呈低水平表达,而从再灌注30 min开始在I/R组和Lid组肠组织中TNF-α呈持续的高表达状态,但这两组中ICAM-1的表达呈缓慢上升趋势;通过测定TNF-α、ICAM-1表达的光密度值(OD值)在组间比较,I/R组和Lid组各时间点测得值均高于Sham组的相应测得值,差异有统计学意义(P<0.05),其中两个指标均以I/R组的测得值为最高,差异有显著性(P<0.05),仅ICAM-1于30、60min表达的OD值在I/R组和Lid组间比较差异无统计学意义(P>0.05).结论 幼鼠再灌注肠组织中TNF-α和ICAM-1的表达增加,利多卡因通过下调肠组织中TNF-α、ICAM-1的表达而对生长发育期动物肠组织I/R损伤有保护作用.

Objective To explore the effects of lidocaine on the expressions of tumor necrosis factor-a (TNF-α) and intercellular adhesion molecule-1 (ICAM-1) in intestines subjected to ischemia/reperfusion (I/R) injury.Methods In this study,48 Sprague-Dawley (SD) rats aged 4 weeks old were randomly divided into 3 groups according to their treatment.In the I/R group,the abdominal cavity of the rats was opened,and the intestinal I/R injury was induced by 60 minutes' superior mesenteric artery occlusion followed by reperfusion for 30 min,60 min,90 min and 120 min.In the lidocaine treatment group,the rats were subjected to intestinal I/R injury,and administered with Lidocaine by femoral vein injection before reperfusion.The sham operated rats were only performed laparotomy.The intestines were harvested for immunohistochemical study to explore the expressions of TNF-α and ICAM-1.Results The sham operated rats had low TNF-α and ICAM-1 expressions.After I/R injury,the expressions of TNF-α and ICAM-1 in intestine were significantly elevated,but Lidocaine treatment inhibited the I/R induced up-regulation of TNF-α and ICAM-1 (all P<0.05).The intestinal expression of ICAM-1 remained unchanged in 30 min and 60 rnin after I/R (all P>0.05).Conclusions Lidocaine down-regulates the expressions of TNF-α and ICAM-1 in intestine after ischemia/reperfusion injury in young rats.
