目的 分析微创颅骨钻孔引流术治疗化脓性脑膜炎合并硬膜下积液术后硬膜下积液中蛋白含量、白细胞数及颅内压变化情况.方法 回顾性分析2008年8月至2011年8月我科收治的60例因化脓性脑膜炎并发硬膜下积液行微创颅骨钻孔引流术患儿的临床资料.分析术中、术后1、3、7、10d及2周时硬膜下积液标本中白细胞数量、蛋白含量及颅内压力变化情况.结果 60例患儿术后硬膜下积液中蛋白含量、白细胞数量及颅内压力随时间逐渐下降,术后7~10d时逐渐降至一个平台期,此后逐渐稳定.白细胞数与蛋白含量相关系数为0.6,蛋白含量与颅内压力相关系数为0.3,白细胞数与颅内压力相关系数为0.6(P均<0.01).结论 术后白细胞数、蛋白含量及颅内压三者成正相关,术后2周左右三者指标趋于稳定为拔管最佳时机,术后积极应用抗生素冲洗能有效地缩短病程.

Objective To study the changes of protein content,leukocytes number in subdural effusion and intracranial pressure in children after the minimally invasive treatment for subdural effusion complicating purulent meningitis.Methods Sixty cases who were performed with minimally invasive cranial drilling drainage between August 2008 and August 2011 were retrospectively analyzed.Leukocytes number and protein content in subdural effusion and intracranial pressure were detected during operation and 1,3,7,10 days and 2 weeks postoperatively,respectively.Results Protein content and leukocytes number in subdural effusion and intracranial pressure declined postoperatively with time gradually to a platform after 7 to 10 days,and then kept stable.Correlation coefficient for leukocytes number and protein content was 0.6 (P<0.01),correlation coefficient for protein content and intracranial pressure was 0.3 (P<0.01),correlation coefficient for leukocytes number and intracranial pressure was 0.6 (P<0.01).Conclusions Leukocyte count,protein content and the intracranial pressure are linearly correlated.The three parameters tend to be more stable after 2 weeks,so 2 weeks after operation is an optimal timing for extubation.Antibiotics rinse can effectively shorten the duration of symptoms.
