目的 比较球面与非球面人工晶状体对高度近视并发白内障患者视觉质量的影响.方法回顾性病例对照研究.采用随机数字表法,将实施超声乳化白内障手术的高度近视并发性白内障患者46例(46眼)随机分为两组.非球面组23例(23眼),行白内障超声乳化联合非球面人工晶状体植入术;球面组23例(23跟),行白内障超声乳化联合球面人工晶状体植入术.观察患者术后3个月的最佳矫正视力(BCVA)、对比敏感度(CS)及波前像差均方根(RMS)值.结果 非球面组BCVA为4.5±0.12,球面组BCVA为4.3±0.15,两组差异无统计学意义(t=1.521,P>0.05).非球面组在夜视(3.0 c/d、6.0 c/d)及夜视加周边眩光(1.5 c/d、3.0 c/d、6.0 c/d)条件下,对比敏感度值明显优于球面组,差别有统计学意义(P<0.05).在瞳孔直径5.0 mm状态下,非球面组的球差、第四阶像差、第五阶像差、第六阶像差及总体高阶像差均明显低于球面组,差异有统计学意义(P<0.05).结论 非球面人工晶状体可以减少高度近视并发性白内障患者术后的高阶相差尤其是球差,提高对比敏感度,具有比球面人工晶状体更佳的视觉质量.
目的 比較毬麵與非毬麵人工晶狀體對高度近視併髮白內障患者視覺質量的影響.方法迴顧性病例對照研究.採用隨機數字錶法,將實施超聲乳化白內障手術的高度近視併髮性白內障患者46例(46眼)隨機分為兩組.非毬麵組23例(23眼),行白內障超聲乳化聯閤非毬麵人工晶狀體植入術;毬麵組23例(23跟),行白內障超聲乳化聯閤毬麵人工晶狀體植入術.觀察患者術後3箇月的最佳矯正視力(BCVA)、對比敏感度(CS)及波前像差均方根(RMS)值.結果 非毬麵組BCVA為4.5±0.12,毬麵組BCVA為4.3±0.15,兩組差異無統計學意義(t=1.521,P>0.05).非毬麵組在夜視(3.0 c/d、6.0 c/d)及夜視加週邊眩光(1.5 c/d、3.0 c/d、6.0 c/d)條件下,對比敏感度值明顯優于毬麵組,差彆有統計學意義(P<0.05).在瞳孔直徑5.0 mm狀態下,非毬麵組的毬差、第四階像差、第五階像差、第六階像差及總體高階像差均明顯低于毬麵組,差異有統計學意義(P<0.05).結論 非毬麵人工晶狀體可以減少高度近視併髮性白內障患者術後的高階相差尤其是毬差,提高對比敏感度,具有比毬麵人工晶狀體更佳的視覺質量.
목적 비교구면여비구면인공정상체대고도근시병발백내장환자시각질량적영향.방법회고성병례대조연구.채용수궤수자표법,장실시초성유화백내장수술적고도근시병발성백내장환자46례(46안)수궤분위량조.비구면조23례(23안),행백내장초성유화연합비구면인공정상체식입술;구면조23례(23근),행백내장초성유화연합구면인공정상체식입술.관찰환자술후3개월적최가교정시력(BCVA)、대비민감도(CS)급파전상차균방근(RMS)치.결과 비구면조BCVA위4.5±0.12,구면조BCVA위4.3±0.15,량조차이무통계학의의(t=1.521,P>0.05).비구면조재야시(3.0 c/d、6.0 c/d)급야시가주변현광(1.5 c/d、3.0 c/d、6.0 c/d)조건하,대비민감도치명현우우구면조,차별유통계학의의(P<0.05).재동공직경5.0 mm상태하,비구면조적구차、제사계상차、제오계상차、제륙계상차급총체고계상차균명현저우구면조,차이유통계학의의(P<0.05).결론 비구면인공정상체가이감소고도근시병발성백내장환자술후적고계상차우기시구차,제고대비민감도,구유비구면인공정상체경가적시각질량.
Objective To compare the effect of an aspheric versus spherical intraocular lens (IOL) on the optical quality of high myopia patients with complicated cataract IOL implantation.Methods In a retrospective study,46 patients (46 eyes) were randomly selected to undergo phacoemulsification and implanted with either aspheric IOLs (test group,23 eyes) or spherical IOLs (control group,23 eyes).BCVA,contrast sensitivity and total higher order wavefront aberrations were assessed 3 months after cataract surgery.Results The difference in BCVA between the two groups was not significant.In an examination of contrast sensitivity under nighttime and nighttime with periglare conditions,patients implanted with aspheric IOLs performed significantly better than patients with spherical IOLs (P<0.05).For a 5.0 mm optical zone,there was a significant reduction in spherical aberrations,RMS4,RMS5,RMS6 and RMSh in eyes implanted with aspheric IOLs (P<0.05).Conclusion This study shows that the optical quality of high myopia and cataractous eyes implanted with aspheric IOLs is significantly better than that with spherical IOLs.
