目的 探讨自体外周血干细胞移植对糖尿病下肢周围神经损害的影响.方法 应用粒细胞集落刺激因子600 μg每13皮下注射5 d,第5天采集外周血干细胞并制成悬液,将干细胞悬液肌内注射入患者双下肢.于移植后3个月分别检测患者胫、腓感觉及运动神经的变化.结果 对48条下肢进行了有效评价,胫运动神经的潜伏期缩短,治疗前为(4.91 ±1.42)ms,治疗后为(4.56±1.47)ms(P<0.05).胫感觉神经传导速度加快,治疗前为(10.23±16.51)m/s,治疗后为(31.33±13.50)m/s(P<0.05).胫感觉神经波幅增加,治疗前为(0.18±0.34)nlv,治疗后为(0.78±0.93)mv(P<0.05).而胫运动神经波幅、胫运动神经传导速度无明显改善.腓运动神经的潜伏期缩短,治疗前为(4.18 ±2.92)ms,治疗后为(3.81±2.93)ms(P<0.05).腓运动神经传导速度加快,治疗前为(37.45±15.59)m/s,治疗后为(40.69 ±15.96)m/s(P<0.05).腓感觉神经传导速度加快,治疗前为(30.89 ±20.31)m/s,治疗后为(43.23±17.36)m/s(P<0.05).结论 自体外周血干细胞移植明显改善糖尿病下肢周围神经损害患者的胫感觉神经、腓感觉及运动神经功能.

목적 탐토자체외주혈간세포이식대당뇨병하지주위신경손해적영향.방법 응용립세포집락자격인자600 μg매13피하주사5 d,제5천채집외주혈간세포병제성현액,장간세포현액기내주사입환자쌍하지.우이식후3개월분별검측환자경、비감각급운동신경적변화.결과 대48조하지진행료유효평개,경운동신경적잠복기축단,치료전위(4.91 ±1.42)ms,치료후위(4.56±1.47)ms(P<0.05).경감각신경전도속도가쾌,치료전위(10.23±16.51)m/s,치료후위(31.33±13.50)m/s(P<0.05).경감각신경파폭증가,치료전위(0.18±0.34)nlv,치료후위(0.78±0.93)mv(P<0.05).이경운동신경파폭、경운동신경전도속도무명현개선.비운동신경적잠복기축단,치료전위(4.18 ±2.92)ms,치료후위(3.81±2.93)ms(P<0.05).비운동신경전도속도가쾌,치료전위(37.45±15.59)m/s,치료후위(40.69 ±15.96)m/s(P<0.05).비감각신경전도속도가쾌,치료전위(30.89 ±20.31)m/s,치료후위(43.23±17.36)m/s(P<0.05).결론 자체외주혈간세포이식명현개선당뇨병하지주위신경손해환자적경감각신경、비감각급운동신경공능.
Objective To investigate the effect of transplantation of autologous peripheral blood stem cells on diabetic peripheral neuropathy of the patients with diabetic lower limb arteriosclerosis obliterans.Methods 24 patients,rhG-CSF 600 μg/d for 5 days.On the fifth day,PBSC were collected.Sooner the PBSC were intramuscularly injected into the two limbs of patients.To do nerves and muscle electrophysiology examination of anticnemion and fibula nerves before and three months after autologous peripheral blood stem cells.Results Nerves and muscle electrophysiology examination of 38 lower limbs of 24 patients showed that latent period of.anticnemion nervimotor was shortened,(4.91±1.42) ms prior treatment,(4.56±1.47)ms,post-treatment (P<0.05).Anticnemion sensory nerves conduction velocity and wave amplitude significantly increased,(10.23±16.51)m/s and (0.18±0.34) mv prior treatment,(31.33±13.50) m/s and (0.78±0.93)mv post-treatment,both of them P<0.05.Latent period of fibula nervimotor was shortened too,(4.18±2.92) ms prior treatment,(3.81±2.93)ms post-treatment (P<0.05).Fibula nervimotor and sensory nerves conduction velocity significantly increased,(37.45±15.59)m/s prior treatment,(40.69±15.96) m/s post-treatment (P<0.05)and (30.89±20.31)m/s prior treatment,(43.23±17.36)m/s post-treatment(P<0.05).Conclusion Transplantation of autologous peripheral blood stem cells significantly improved the damage of diabetic peripheral neuropathy.
