目的 探讨原发性开角型青光眼(POAG)和原发性慢性闭角型青光眼(CPACG)患者频域光学相干断层扫描(OCT)检测的视网膜神经纤维层(RNFL)厚度与视野光敏感度的相关性.方法 临床观察研究.采用RTV100-20CT和OCTOPUS101视野计对POAG患者22例(22眼)及CPACG患者22例(22眼)进行检测,将两组患者RNFL厚度及视野光敏感度值进行比较,应用Pearson积矩相关分析法确定平均RNFL厚度与平均敏感度(MS)、上方和下方平均RNFL厚度与相对应半侧视野区平均敏感度的相关性.结果 POAG组平均RNFL厚度、上方平均RNFL厚度及下方平均RNFL厚度分别为(91.19±15.85) μm、(94.93±18.76) μm、(87.82±15.65) μm,CPACG组相应的RNFL厚度分别为(88.22±13.01) μm、(90.41±14.16) μm、(86.49±16.63) μm,两组患者各RNFL参数的比较差异均无统计学意义(P>0.05).POAG组平均敏感度(MS)、上方半侧视野平均敏感度及下方半侧视野平均敏感度分别为(19.80±6.22) dB、(18.64±6.23) dB、(20.87±6.46) dB,CPACG组相应的光敏感度值分别为(16.36±3.87) dB、(15.62±4.95) dB、(17.19±5.76) dB,两组患者MS的比较差异有统计学意义(P<0.05),上、下方半侧视野平均敏感度的比较差异均无统计学意义(P>0.05).POAG组平均RNFL厚度、上方平均RNFL厚度及下方平均RNFL厚度与视野相对应区域光敏感度值间的相关系数分别为:0.619、0.562、0.577(P<0.01),CPACG患者平均RNFL厚度、上方平均RNFL厚度及下方平均RNFL厚度与视野相应区域光敏感度值间的相关系数分别为0.822、0.872、0.802 (P <0.01),两组患者平均RNFL厚度与MS的相关系数和下方平均RNFL厚度与上方半侧视野平均敏感度的相关系数的差异均无统计学意义(P>0.05),两组患者上方平均RNFL厚度与下方半侧视野平均敏感度的相关系数的差异有统计学意义(P<0.05).结论 POAG结构与功能呈中度正相关,CPACG结构与功能呈高度正相关;相对于POAG,CPACG上方平均RNFL厚度与下方半侧视野平均敏感度具有更好的相关性.

Objective To explore the correlations between retinal nerve fiber layer (RNFL) thickness measured with spectral domain optical coherence tomography (OCT) and visual field light sensitivity in POAG and CPACG.Methods Twenty-two patients (22 eyes) with POAG and 22 patients (22 eyes) with CPACG were examined by using RTV100-2OCT and OCTOPUS101 visual field analyzer.The RNFL thickness and visual field light sensitivity in the two groups of patients were compared.Correlations between RNFL thickness and visual field light sensitivity were determined by Pearson product moment correlation analysis.Results In POAG group,the average RNFL thickness,superior average RNFL thickness and inferior average RNFL thickness were (91.19±15.85)μm,(94.93±18.76)μm,(87.82±15.65)μm,respectively; in CPACG group,the corresponding RNFL thickness were (88.22 ± 13.01)μm,(90.41 ± 14.16)μm,(86.49 ± 16.63)μm,respectively; differences of RNFL parameters was not significant between POAG group and CPACG group (P >0.05).In POAG group,mean sensitivity (MS),superior hemi-visual field average sensitivity and inferior hemi-visual field average sensitivity were (19.80±6.22)dB,(18.64±6.23)dB,(20.87±6.46)dB,respectively; in CPACG group,the corresponding sensitivity were (16.36±3.87)dB,(15.62±4.95)dB,(17.19±5.76)dB,respectively; differences of MS was significant between POAG group and CPACG group (P <0.05),differences of superior and inferior hemi-visual field average sensitivity was not significant between POAG group and CPACG group (P >0.05).In POAG group,average RNFL thickness,superior and inferior average RNFL thickness were correlated with average sensitivity of the corresponding visual field with the significant correlation coefficient 0.619,0.562,0.577,respectively (P <0.01); in CPACG group,average RNFL thickness,superior and inferior average RNFL thickness were correlated with average sensitivity of the corresponding visual field with the significant correlation coefficient 0.822,0.872,0.802,respectively (P <0.01).The differences of correlation coefficient between average RNFL thickness,inferior average RNFL thickness with average sensitivity of the corresponding visual field was not significant in the two groups (P >0.05).The differences of correlation coefficient between superior average RNFL thickness with inferior hemi-visual field average sensitivity was significant in the two groups (P <0.05).Conclusions POAG eyes have moderate positive structure-function correlations.CPACG eyes have high positive structure-function correlations.Compared with POAG eyes,CPACG has better correlations between superior average RNFL thickness and inferior hemi-visual field average sensitivity.
