目的 观察青少年近视患者配戴单光框架眼镜、渐变多焦镜、硬性透氧性角膜接触镜(Rigid Gas Permeable Contact Lens,RGPCL)调节滞后及近距隐斜的变化情况.方法 选取75例9~14岁中低度近视患者,根据患者的屈光度数、角膜参数、近距隐斜及调节滞后将患者平均分为3组,单光眼镜组、渐变镜组、RGPCL组,分别测量配戴前、配戴1年后的调节滞后和近距隐斜量,比较三种矫正方式两参数的变化量.结果 完成随访的患者61例.戴镜1年后,仅渐变多焦镜组调节滞后量的改变差异有统计学意义(t =2.208,P=0.040),各组近距隐斜变化量差异均无统计学意义(P>0.05).结论 单光眼镜、渐变多焦镜和RGPCL延缓近视进展的机制可能与调节滞后和近距隐斜有关.

Objective To observe the change of accommodative lag and near distance phoria in the adolescent myopia with three different refractive corrections (single focus glasses,progressive lenses and rigid gas permeable contact lens).Methods To choose 75 patients of 9-14 years old,according to the patients' refraction,corneal parameters,near distance phoria and accommodative lag,these patients were equally divided into three groups:respectively wore single focus glasses,progressive lenses and RGPCL.The accommodative lag and near distance phoria before and after 1 year were examined.The two parameters' change in three groups was compared.Results After 1 year,61 patients finally were followed.The accommodative lag was statistically changed only in progressive lenses group (t =2.208,P =0.040).The amount of near distance phoria in each group was not statistically changed (P >0.05).Conclusions The mechanism of slowing down myopic progression with 3 different refractive corrections may be related to accommodative lag and near distance phoria.
