目的探讨羟基磷灰石(Hydroxyapatite,HA)义眼座,植入术后暴露的原因及处理.方法对40例义眼座植入患者进行随访分析.术后发生义眼座暴露8例(20％).针对暴露的程度及原因采取不同的治疗方法.结果 3例轻度患者经保守治疗愈合,4例中重度患者经羊膜移植愈合,1例重度患者经取出义眼座后愈合,随诊3月至3年无复发.结论羟基磷灰石义眼座植入术后暴露的原因与多种因素有关,恰当地处理可以使之恢复.羊膜移植是治疗中重度结膜裂开、眼座暴露的有效方式.

Objective To analyze the causes and management of exposed hydroxyapatite (HA) artificial eyes after implantation.Methods Forty patients were performed artificial orbit implantation and followed up.Artificial orbital implant exposure was observed in 8 of the 40 cases (20.0％).Different therapies were applied to different cases in accordance to the causes.Results Three cases of mild patients by conservative treatment healing,4 cases of moderate and severe patients by amniotic membrane transplantation healing,1 case of severe patients after the removal of artificial eye pedestal healing,and no recurrence was observed after 3 months to 3 years follow up.Conclusions Several factors may contribute to the implant exposure; proper management is effective for the exposure of hydroxyapatite.Amniotic membrane transplantation is an effective way in the treatment of moderate and severe exposure of hydroxyapatite.
