目的评价Amaris准分子激光系统的经角膜上皮的激光角膜切削术治疗轻中度近视及近视散光的安全性及有效性.方法前瞻性研究.对2012年7月至2013年4月在天津市眼科医院屈光中心行准分子激光手术的轻中度近视及近视散光患者共25例(42只眼),分为实验组15例(22只眼),行经角膜上皮的激光角膜切削术(transepithelial photorefractive keratectomy,tPRK);对照组10例(20只眼),行乙醇法准分子激光上皮瓣下角膜磨镶术(laser epithelial keratomileusis,LASEK).记录术后1～3d患者主观疼痛指数及手术后3个月裸眼视力、显然验光结果及haze程度.结果手术后tPRK组在术后第二天患者主观疼痛指数小于LASEK组(1.91 vs 3.27),差异有统计学意义(t=2.179,P=0.042),术后第一天及第三天疼痛指数、术后3个月时裸眼视力、显然验光结果两组比较差异均无统计学意义(t值分别为0.157、0.488、0.321及0.165,P值分别为0.877、0.631、0.752、0.871),Haze程度两组比较无统计学意义(x2=0.286,P=0.59).结论 Amaris准分子激光系统的tPRK术式简化了手术程序,其治疗轻中度近视及近视散光是安全有效的.

Objective To assess the effectiveness,safety of transepithelial photorefractive keratectomy (tPRK) using the Amaris laser platform.Methods In this prospective study,22 myopic eyes with or without astigmatism were treated by tPRK (study group) and compared with 20 eyes treated by conventional laser epithelial keratomileusis (control group).Postoperative pain score,uncorrected distance visual acuity (UDVA),manifest refraction,and haze were analyzed.Results The mean subjective postoperative pain score at 2 day after surgery was 1.91 in the study group and 3.27 in the control group (t =2.179,P =0.042),there was no statistically significant difference in postoperative pain score at 1 day and 3 days,UDVA and manifest refraction between the groups (t =0.157,0.488,0.321,0.165 respectively,P =0.877,0.631,0.752,0.871 respectively),haze level had no statistically significant difference between the groups(x2=0.286,P =0.59).Conclusions Transepithelial PRK for mild to moderate myopia with or without astigmatism is safe and easier to perform with Amaris laser platform.
