目的 以光学相干断层成像(optical coherence tomography,OCT)分析青年人正视眼和不同屈光度的视网膜神经纤维层(retinal nerve fiber layer,RNFL)厚度及视盘参数的差异.方法 临床病例系列研究.对2012年3月至2013年4月在广州医学院志愿参加研究的青年人110名220只眼.全部使用OCT的3D模式进行正视眼和不同程度近视眼的RNFL厚度及视盘参数检测.结果 18~27岁的青年人110名.中高度近视与正视眼相比,全周平均和鼻侧、上方、下方RNFL厚度都明显变薄,颞侧明显变厚,差异有统计学意义(P<0.05).以上各部位的RNFL厚度在高度近视眼比正视眼相差分别为8.53 μm (7.77%),14.71 μm (19.04%),11.96 μm (8.97%),18.2 μm (12.78%)和-10.37 μm(-12.07%).各种程度的近视眼与正视眼相比,视盘面积等各项参数均变小,除盘缘面积和盘缘容积外差异均有统计学意义(P<0.05).结论 近视眼的RNFL厚度比正视眼薄,鼻侧变化最早也最大.上方和下方相继变薄,同时颞侧在增厚.近视眼视盘参数均变小.

Objective To study the characteristics and clinical significance of retinal nerve fiber layer (RNFL) thickness and optic nerve head (ONH) parameters along with an increase of myopia degree.Methods According to the myopic degree,110 subjects were divided into 4 groups:normal,slight,medium and high myopic groups.Optical coherence tomography (OCT) was used to measure the RNFL thickness and ONH parameters.Each subject had 3D scans on the ONH with 6× 6mm (512×128).The mean and different quadrants RNFL thickness and ONH parameters in different groups were analyzed.Results Compared with normal groups,the mean RNFL thicknesses in all myopic groups were significantly thinner.The change occurred first in nasal quadrant.RNFL thicknesses in high myopic groups were significantly thinner in nasal,inferior and superior quadrant.However,temporal quadrant was significantly thicker.Compared with normal groups,ONH parameters in every myopic group were smaller.Except Rim Area and Rim Volume,all parameters were significantly smaller.Conclusions The mean RNFL thickness in myopic groups is thinner and thinner along with an increase of myopia degree.Different from others,temporal quadrant is thicker.Except Rim Area and Rim Volume,all ONH parameters are significantly smaller.Above mentioned features have the clinical significance for diagnosing disease.
