目的观察糖尿病患者应用角膜缘干细胞移植联合适宜浓度丝裂霉素C治疗翼状胬肉的临床疗效.方法临床病例系列研究.对于2007年9月至2012年12月在内蒙古民族大学附属医院眼科就诊的患有Ⅱ型糖尿病的63例翼状胬肉患者(79只眼)行自体角膜缘干细胞移植联合0.1 mg/ml丝裂霉素C治疗,随访6～48个月,观察术前、术后视力与屈光状态的变化情况,以及角膜修复、植片愈合和复发情况.结果 Prabhasawat分级:1级愈合的58只眼占73.42％,2级11只眼占13.92％,3级8只眼占10.13％,4级2只眼占2.53％.术后患者裸眼视力均有不同程度提高,散光度数均较术前明显下降,差异有统计学意义(P＜0.05).植片全部成活,无明显并发症发生.结论自体角膜缘干细胞移植联合适宜浓度的丝裂霉素C是治疗糖尿病患者翼状胬肉的理想术式.

Objective To investigate the clinical outcome of limbal stem cells transplantationcombined with mitomycin C treatment of pterygium in diabetic patients.Methods Sixty-three cases(79 eyes) with diabetic pterygium were treated with autologous limbal stem cell conjuctiva limbaltransplantation and mitomycin C at 0.1mg/ml.The diabetics should be followed up for 6-48 monthsand then preoperative status,postoperative vision,the changes of refractive state and the cases of cor-nea repair,implant healing and recurrence were observed.Results Based on four levels of Pmbha-sawat classification,Level one healing was 58 eyes (73.42％),Level two healing 11 eyes (13.92％),Level three healing 8 eyes (10.13％),and Level four healing 2 eyes (2.53％).The postoperative nakedvision of all patients had different degree of improvement and astigmatism degree declined obviouslythan preoperative,there was statistic significant (P ＜0.05).All grafts survived and no obvious complica-tions occurred.Conclusions The autologous limbal stem cells transplantation combined with appropri-ate concentration mitomycin C is the ideal operation for treatment of pterygium patients with diabetes.
