目的 探讨前葡萄膜炎患者的临床特点、治疗效果、视力预后和诊治中存在的问题.方法 回顾性系列病例研究.自2012年10月至2013年6月间,收集于天津市眼科医院就诊的各类前葡萄膜炎患者109例,男52例,女57例;平均发病年龄(41±15)岁.通过临床检查、实验室检查和诊治观察,对其临床类型、疗效、并发症和视力预后进行分析.结果 109例患者中,急性前葡萄膜炎39例(35.8％),慢性前葡萄膜炎15例(13.8％),复发性前葡萄膜炎55例(50.5％).根据病因分为特发性前葡萄膜炎52例(47.7％),病毒性前葡萄膜炎11例(10.1％),Fuchs综合征5例(4.6％),青光眼睫状体炎综合征3例(2.8％);合并血清阴性脊柱关节病22例(20.2％)、糖尿病8例(7.3％)、类风湿性关节炎4例(3.7％)和幼年性关节炎3例(2.8％),结核病1例(0.9％).各种并发症发生率为34.9％ (38例),轻度视力损害(低于0.5)发生率为7.3％(8例),中重度视力损害(低于0.3)为6.4％(7例),致盲率(低于0.05)为1.8％(2例).结论 前葡萄膜炎临床类型多样,特发性前葡萄膜炎、血清阴性脊柱关节病伴发前葡萄膜炎及病毒性前葡萄膜炎为常见类型.急性前葡萄膜炎易复发,但视力预后较好,慢性或顽固性炎症常导致严重视力损害或致盲.
目的 探討前葡萄膜炎患者的臨床特點、治療效果、視力預後和診治中存在的問題.方法 迴顧性繫列病例研究.自2012年10月至2013年6月間,收集于天津市眼科醫院就診的各類前葡萄膜炎患者109例,男52例,女57例;平均髮病年齡(41±15)歲.通過臨床檢查、實驗室檢查和診治觀察,對其臨床類型、療效、併髮癥和視力預後進行分析.結果 109例患者中,急性前葡萄膜炎39例(35.8％),慢性前葡萄膜炎15例(13.8％),複髮性前葡萄膜炎55例(50.5％).根據病因分為特髮性前葡萄膜炎52例(47.7％),病毒性前葡萄膜炎11例(10.1％),Fuchs綜閤徵5例(4.6％),青光眼睫狀體炎綜閤徵3例(2.8％);閤併血清陰性脊柱關節病22例(20.2％)、糖尿病8例(7.3％)、類風濕性關節炎4例(3.7％)和幼年性關節炎3例(2.8％),結覈病1例(0.9％).各種併髮癥髮生率為34.9％ (38例),輕度視力損害(低于0.5)髮生率為7.3％(8例),中重度視力損害(低于0.3)為6.4％(7例),緻盲率(低于0.05)為1.8％(2例).結論 前葡萄膜炎臨床類型多樣,特髮性前葡萄膜炎、血清陰性脊柱關節病伴髮前葡萄膜炎及病毒性前葡萄膜炎為常見類型.急性前葡萄膜炎易複髮,但視力預後較好,慢性或頑固性炎癥常導緻嚴重視力損害或緻盲.
목적 탐토전포도막염환자적림상특점、치료효과、시력예후화진치중존재적문제.방법 회고성계렬병례연구.자2012년10월지2013년6월간,수집우천진시안과의원취진적각류전포도막염환자109례,남52례,녀57례;평균발병년령(41±15)세.통과림상검사、실험실검사화진치관찰,대기림상류형、료효、병발증화시력예후진행분석.결과 109례환자중,급성전포도막염39례(35.8％),만성전포도막염15례(13.8％),복발성전포도막염55례(50.5％).근거병인분위특발성전포도막염52례(47.7％),병독성전포도막염11례(10.1％),Fuchs종합정5례(4.6％),청광안첩상체염종합정3례(2.8％);합병혈청음성척주관절병22례(20.2％)、당뇨병8례(7.3％)、류풍습성관절염4례(3.7％)화유년성관절염3례(2.8％),결핵병1례(0.9％).각충병발증발생솔위34.9％ (38례),경도시력손해(저우0.5)발생솔위7.3％(8례),중중도시력손해(저우0.3)위6.4％(7례),치맹솔(저우0.05)위1.8％(2례).결론 전포도막염림상류형다양,특발성전포도막염、혈청음성척주관절병반발전포도막염급병독성전포도막염위상견류형.급성전포도막염역복발,단시력예후교호,만성혹완고성염증상도치엄중시력손해혹치맹.
Objective To investigate the clinical characters,effects of treatment,prognosis of visual acuity and the problems in the treatment of anterior uveitis patients.Methods The retrospective case series study.Collecting 109 cases of anterior uveitis from October 2012 to June 2013 in Tianjin Eye Hospital,there were 52 cases of male and 57 cases of female.The mean age was (41± 15) years old.Through clinical examination,laboratory tests and treatment observation,the clinical characters,effects of treatment,complications and prognosis of the anterior uveitis were analyzed.Results In 109 patients,there were 39 cases (35.8％) with acute anterior uveitis,15 (13.8％) with recurrent anterior uveitis and 55 (50.5％) with chronic anterior uveitis.There were 52 cases (47.7％) with idiopathic uveitis,11 (10.1％) with herpetic uveitis,5 (4.6％) with Fuchs syndrome and 3 (2.8％) with glaucomatocyclitic crisis,22 (20.2％) with seronegative spondyloarthropathy,8 (7.3％) with diabetes mellitus,4 (3.7％) with rheumatoid arthritis and 3 (2.8％) with juvenile idiopathic arthritis based on the etiological classification.The rate of complications was 34.9％ (38 cases),the mild visual impairment (less than 0.5) was 7.3％ (8 cases),the moderate and severe visual impairment (less than 0.3) was 6.4％ (7 cases),and the blindness (less than 0.05) was 1.8％ (2 cases).Conclusions The types of anterior uveitis are varied.Idiopathic anterior uveitis,seronegative spondyloarthropathy associated uveitis and herpetic uveitis are the common entities.Acute anterior uveitis is easy to relapse,but vision prognosis is often good,but the chronic inflammation often leads to the severe visual impairment or blindness.
