目的探讨光学相干断层扫描(optical coherence tomography,OCT)在白内障超声乳化吸除术前眼底评估中的应用价值.方法选择2012年10月至2013年4月在黔西南州人民医院眼科就诊的老年性白内障患者415例(500只眼),行白内障超声乳化及人工晶状体植入术前除进行眼科常规检查外,采用海德堡公司生产的Spectralis OCT进行黄斑区OCT检查,为评价术后视功能恢复提供客观依据,并统计常规眼底镜检查的漏诊率.结果拟行白内障超声乳化及人工晶状体植入术的415例(500只眼),经直接及间接眼底镜检查基本或隐约可见眼底,未发现明显异常,均可成功获取OCT图像.在术前行黄斑OCT检查,有28例(32只眼)检出不同程度的黄斑病变.结论 OCT在白内障超声乳化吸除术前检查中,对黄斑病变检查灵敏度高,能有效提高白内障术前黄斑病变的检出率,有利于对白内障手术,做好术前交代和预后效果的综合判断,避免医患纠纷的发生.

Objective To explore the value of optical coherence tomography (OCT) for fundus evaluation before cataract surgery.Methods From October 2012 to April 2013,415 cases (500 eyes) of age-related cataract with normal or vaguely fundus were examined using direct and indirect ophthalmoscopy.Addition to routine eye examination,Heidelberg Spectralis macular OCT examination was performed for the evaluation of postoperative recovery of visual function before phacoemulsification and intraocular lens implantation surgery.Four hundred cases (400 eyes) or only IOSS of foveaI reflex under direct and indirect funduscopes were selected for study.Results Direct and indirect ophthalmoscopy examined basic or vaguely visible fundus for those who would receive phacoemulsification and intraocular lens implantation surgery,no obvious abnormalities were found,and OCT images could be successfully obtained,macular OCT examination of which 28 cases (32 eyes) detected in varying degrees of macular degeneration.Conclusions Preoperative OCT examination before cataract phacoemulsification for checking macular degeneration has high sensitivity,it can improve the detection rate of macular degeneration,and is beneficial to the overall judgment for cataracts prognosis to avoid the occurrence of medical disputes.
