目的了解吉林省早产儿视网膜病变(ROP)的患病情况及相关危险因素.方法对2007年7月至2012年5月在吉林大学第一医院儿科收治的孕周＜37周或出生体重≤2 000 g的早产儿或超过以上范围但患有严重疾病的2 352例早产儿应用RetcamⅡ进行ROP筛查.结果 2 352例早产儿中患不同程度ROP者479例,ROP患病率为20.37％ (479/2 352);孕周越短、出生体重越低,ROP患病率越高.对可能影响ROP患病率的24种因素进行单因素分析,结果显示孕周、出生体重、吸氧、贫血、输血史、先心病、肺炎、窒息、急性呼吸窘迫综合征(ARDS)、败血症、胎膜早破、母乳喂养与ROP患病率密切相关,性别、多胎、剖宫产、前置胎盘、胎盘早剥、脐带绕颈、试管婴儿、感染、黄疸、紫癜、母孕时妊高症及阴道流血的有无与ROP患病率无相关性; 进一步行Logistic多元回归分析,显示孕周、出生体重、吸氧史、输血史、败血症、母乳喂养和胎膜早破与ROP患病率相关,是ROP发生的高危因素.结论在研究中ROP的患病率为20.37％(479/2 352),其发生与多因素有关.孕周、出生体重、吸氧史、输血史、败血症、母乳喂养和胎膜早破是影响ROP患病率的高危因素.

Objective To learn the screening results and risk factors for retinopathy of prematurity (ROP) in the preterm infants in Jilin Province.Methods From July 2007 to May 2012,2352premature infants with birth weight ≤2000g or gestational age 37 weeks or the ones having severe systemic disease in the First Hospital of Jilin University were screened for ROP with Retcam Ⅱ.Results Of 479 infants were diagnosed as retinopathy at different degrees among 2352 premature infants,the incidence of retinopathy was 20.37％ (479/2352); the infants with small gestational age and low birth weight had high incidence of retinopathy; 24 probable effect factors of retinopathy were involved into single factor analysis,and the results indicated that gestational age,birth weight,oxygen duration,anemia,blood transfusion,pneumonia,asphyxia,ARDS,congenital heart disease,breast-feeding,septicemia and premature rupture of membrane were related to incidence of retinopathy closely.There was no correlation between gender,infection,polyembryony,caesarean birth,hypertension and vaginal bleeding during pregnancy,test-tube baby,purpura,placenta praevia,placental abruption,cord around the neck,jaundice neonatal acidosis and incidence of retinopathy; multivariate logistic regression analysis showed that gestational age,birth weight,oxygen duration,blood transfusion,septicemia,breast-feeding and premature rupture of membrane were the most relative factors and they were high risk factors of retinopathy.Conclusions The incidence of ROP in the present study is 20.37％ (479/2352).Gestational age,birth weight,oxygen duration,blood transfusion,septicemia,breast-feeding and premature rupture of membrane are high risk factors to affect ROP incidence.
