目的探讨经升主动脉注射鱼精蛋白对婴幼儿体外循环心脏直视手术肺泡灌洗液中细胞计数的影响,进一步证实其肺保护作用.方法随机抽取120例(年龄≤10个月,体质量≤8 kg)行体外循环心脏直视手术患儿,分为实验组(升主动脉组,60例)和对照组(中心静脉组,60例),比较两组给鱼精蛋白前后肺泡灌洗液中细胞数目及分类的变化.结果实验组及对照组给鱼精蛋白前,肺泡灌洗液中细胞数目及分类比较差异均无统计学意义(P均＞0.05);实验组经升主动脉注射鱼精蛋白1h肺泡灌洗液中性粒细胞及总细胞计数较对照组显著升高(t值分别为2.568、1.984,P均＜0.05).结论经升主动脉注射鱼精蛋白能够减轻婴幼儿体外循环心脏直视手术肺部中性粒细胞渗出,减轻肺损伤,是一种良好的肺保护措施.

Objective To investigate the effect of injecting protamine on the counts of cell on infants undergoing opening heart operation by cardiopulmonary bypass,further confirmed its lung protective effect.Methods One hundred and twenty infants (age ≤10 month,weight≤8 kg) underwent opening heart operation by cardiopulmonary bypass were randomly divided into experiment group(receiving intraaortic,n =60) and control groups (intravenous protamine group,n =60).Total cell and neutrophil in the bronchoalveolar lavage fluid were counted before and after injecting protamine.Results There were no significant differences in total cell number and neutrophil in the bronchoalveolar lavage fluid between two groups before injecting protamine (P ＞ 0.05).The number of total cell and neutrophil in the bronchoalveolar lavage fluid of intravenous protamine groups were markedly higher compared with intraaortic protamine groups for 1 h after injecting protamine (t =1.984,2.568 respectively,P ＜0.05).Conclusion Intraaortic protamine can decrease the number of total cell and neutrophil in the bronchoalveolar lavage fluid,and less the lung injury.It is the good way for lung protection in infants of opening heart operation by intraaortic protamine.
