目的 比较剪切波弹性成像与PET/CT 18 F-FDG显像两种检查方法及两者联合对甲状腺结节良恶性的诊断价值.方法 以106个甲状腺结节为研究对象,均行剪切波弹性成像及PET/CT18 F-FDG显像检查,以病理结果为标准,统计两种方法及两者联合对甲状腺结节良恶性诊断的灵敏性、特异性、准确性、阳性预测值及阴性预测值.结果 106个结节中74个为恶性结节,32个为良性结节,剪切波弹性成像的灵敏性、特异性、准确性、阳性预测值及阴性预测分别为87.8%、75.0%、84.0%、89.0%、72.7%,PET/CT18F-FDG显像分别为81.1%、65.6%、76.4%、84.5%、60.0%,两者联合分别为90.5%、84.4%、88.7%、93.1%、79.4%.结论 剪切波弹性成像对甲状腺结节良恶性的诊断价值与PET/CT 18 F-FDG显像相比无明显统计学意义,两种方法各有优势,将两者联合应用,可以提高甲状腺结节术前诊断准确性.

Objective To compare the diagnostic value of shear wave elastography and PET/CT 18F-FDG imaging for thyroid nodules and to study their combined diagnostic values.Methods 106 thyroid nodules were studied and they were characterized by shear wave elastography and PET/CT 18 F-FDG examination respectively.The results were confirmed by pathology and then the diagnostic sensitivity,specificity,accuracy,positive and negative predictive value were got by the statistics for the two methods and their combination on benign and malignant thyroid nodules.Results Of 106 nodules,74 were malignant nodules,32 were benign nodules.The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of shear wave elastography was 87.8%,75.0%,84.0%,89.0%,72.7%,PET/CT was 81.1%,65.6%,76.4%,84.5%,60.0%,the combination of the two methods was 90.5%,84.4%,88.7%,93.1%,79.4%,respectively,conducted x2 test,P >0.05.Conclusions The diagnostic value of shear wave elastography and the value of PET/CT 18 F-FDG imaging for benign and malignant thyroid nodules showed no statistically significance.Both methods have advantages and the combination of the two applications can improve the accuracy of pre-operative diagnosis of thyroid nodules.
