目的 检测变应性鼻炎(allergic rhinitis,AR)、非变应性鼻炎(non-allergic rhinitis,NAR)患者和健康对照组在鼻分泌物及血清中的免疫球蛋白游离轻链(free light chain,FLC)的表达水平,初步探讨FLC可能参与AR及NAR发病的免疫学机制.方法 选取2009年9-12月就诊的AR 及NAR患者各30例以及健康对照组30例,所有入组病例均进行过敏原皮肤点刺试验(skin prick tests,SPT)及血清特异性IgE检测(specific IgE,sIgE).对入组的所有患者进行鼻部症状的视觉模拟量表评分(visual analogue scale,VAS),包括阵发性喷嚏、鼻涕、鼻塞及鼻痒,采集患者外周血及鼻腔分泌物,检测其中的总IgE、嗜酸粒细胞阳离子蛋白(eosinophil cationic protein,ECP)、肥大细胞类胰蛋白酶(mast cell tryptase,MCT)、κFLC、λFLC蛋白表达水平.结果 AR及NAR组患者喷嚏、鼻涕、鼻塞、鼻痒的VAS评分差异无统计学意义(t值分别为1.189、0.741、0.758、0.797,P值均>0.05).血清中κFLC、λFLC、ECP、MCT蛋白表达水平在NAR组均高于健康对照组(P值均<0.05);λFLC蛋白表达水平在NAR组高于AR组,差异有统计学意义(P<0.05);κFLC、总IgE及ECP蛋白表达水平在AR 组高于健康对照组(P值均<0.05).AR组及NAR组鼻分泌物中κFLC、λFLC、ECP及MCT的蛋白表达水平均较健康对照组明显升高(P值均<0.05);ECP和总IgE蛋白表达水平在AR组较NAR组明显升高(P值均<0.05).鼻分泌物中κFLC、λFLC分别与MCT呈正相关趋势(r值分别为0.518、0.484,P值均<0.05).血清中κFLC、λFLC分别与ECP呈正相关趋势(r值分别为0.343、0.342,P值均<0.05).结论 FLC可能参与了AR及NAR的发病,其在AR及NAR的发病过程中可能通过肥大细胞及嗜酸粒细胞参与并发挥一定的免疫学作用.

목적 검측변응성비염(allergic rhinitis,AR)、비변응성비염(non-allergic rhinitis,NAR)환자화건강대조조재비분비물급혈청중적면역구단백유리경련(free light chain,FLC)적표체수평,초보탐토FLC가능삼여AR급NAR발병적면역학궤제.방법 선취2009년9-12월취진적AR 급NAR환자각30례이급건강대조조30례,소유입조병례균진행과민원피부점자시험(skin prick tests,SPT)급혈청특이성IgE검측(specific IgE,sIgE).대입조적소유환자진행비부증상적시각모의량표평분(visual analogue scale,VAS),포괄진발성분체、비체、비새급비양,채집환자외주혈급비강분비물,검측기중적총IgE、기산립세포양리자단백(eosinophil cationic protein,ECP)、비대세포류이단백매(mast cell tryptase,MCT)、κFLC、λFLC단백표체수평.결과 AR급NAR조환자분체、비체、비새、비양적VAS평분차이무통계학의의(t치분별위1.189、0.741、0.758、0.797,P치균>0.05).혈청중κFLC、λFLC、ECP、MCT단백표체수평재NAR조균고우건강대조조(P치균<0.05);λFLC단백표체수평재NAR조고우AR조,차이유통계학의의(P<0.05);κFLC、총IgE급ECP단백표체수평재AR 조고우건강대조조(P치균<0.05).AR조급NAR조비분비물중κFLC、λFLC、ECP급MCT적단백표체수평균교건강대조조명현승고(P치균<0.05);ECP화총IgE단백표체수평재AR조교NAR조명현승고(P치균<0.05).비분비물중κFLC、λFLC분별여MCT정정상관추세(r치분별위0.518、0.484,P치균<0.05).혈청중κFLC、λFLC분별여ECP정정상관추세(r치분별위0.343、0.342,P치균<0.05).결론 FLC가능삼여료AR급NAR적발병,기재AR급NAR적발병과정중가능통과비대세포급기산립세포삼여병발휘일정적면역학작용.
Objective To test the immunoglobulin free light chain (FLC) from nasal secretion and serum of patients with allergic rhinitis(AR) and non-allergic rhinitis(NAR) for the purpose of exploring the possible immunological mechanism.Methods Sixty consecutive patients were selected between September and December in 2009,involving 30 patients with AR and 30 patients with NAR diagnosed by symptoms,signs,skin prick tests (SPT) and specific IgE (sIgE).Thirty volunteers were chosen as healthy control (HC).According to the visual analogue scale (VAS) scores,the nasal symptoms of AR and NAR,including sneeze,nasal discharge,nasal obstruction and nasal itching were compared.ELISA was used to detect the total IgE,eosinophil cationic protein(ECP),mast cell tryptase(MCT),κFLC,λFLC in nasal secretion and serum.The data was analyzed by SPSS 17.0 software.Results There was no statistical difference between AR and NAR group in nasal symptoms (t value was 1.189,0.741,0.758,0.797,respectively,all P > 0.5);In serum,κFLC,λFLC,ECP,MCT increased in NAR group compared to HC group (all P < 0.05);λFLC increased in NAR group compared to AR group(P < 0.05),κFLC,total IgE and ECP was increased in AR group compared to HC group.In nasal secretion,κFLC,λFLC,ECP and MCT increased in AR and NAR groups compared to HC group,and the ECP and total IgE were significantly increased in AR group compared to NAR group (all P < 0.05).In nasal secretion,the FLC revealed a significantly higher correlation trend with MCT (r value was 0.518 and 0.484,all P <0.05),and in serum revealed a significant correlation trend with ECP (r value was 0.343 and 0.342,all P < 0.05).Conclusion Immunoglobulin FLC takes part in the path of physiological process of AR and NAR with the immunological mechanism.
