目的 利用MRI三维模型分析青年健康未育女性盆底肌群的形态及解剖学特点.方法 选取南方医科大学25例青年健康未育女性志愿者,采集其盆腔MRI数据集,利用计算机三维重建软件构建骨盆、盆腔器官及盆底肌群的三维模型,并对肛提肌进行相关指标的测量,包括:肛提肌体积(LVOL),肛提肌板角度(LPA),肛提肌裂孔横径(LH-W)及前后径(LH-L),肛提肌距耻骨联合下缘距离(LSG).结果 (1)成功构建25例MRI三维模型,包括骨盆、盆腔器官及盆底肌群(包括坐骨尾骨肌、肛提肌及其各个组成部分、会阴肌群),该模型形态逼真,直观、立体性强,能清晰反映盆底肌群层次.(2)肛提肌测量结果:LVOL为(34±6) cm3,LPA为(43±4)°,LH-W为(33±4) mm,LH-L为(54±5) mm,LSG左侧为(18.8±2.5) mm,右侧为(18.3±2.5)mm.结论 利用MRI数据集可重建出女性盆底肌群,是研究女性盆底肌群解剖的有效方法;对肛提肌三维模型进行量化分析,可评估盆底功能,有助于理解盆底功能障碍性疾病发病机制,为临床诊断、治疗方法选择及疗效评价提供参考标准.

목적 이용MRI삼유모형분석청년건강미육녀성분저기군적형태급해부학특점.방법 선취남방의과대학25례청년건강미육녀성지원자,채집기분강MRI수거집,이용계산궤삼유중건연건구건골분、분강기관급분저기군적삼유모형,병대항제기진행상관지표적측량,포괄:항제기체적(LVOL),항제기판각도(LPA),항제기렬공횡경(LH-W)급전후경(LH-L),항제기거치골연합하연거리(LSG).결과 (1)성공구건25례MRI삼유모형,포괄골분、분강기관급분저기군(포괄좌골미골기、항제기급기각개조성부분、회음기군),해모형형태핍진,직관、입체성강,능청석반영분저기군층차.(2)항제기측량결과:LVOL위(34±6) cm3,LPA위(43±4)°,LH-W위(33±4) mm,LH-L위(54±5) mm,LSG좌측위(18.8±2.5) mm,우측위(18.3±2.5)mm.결론 이용MRI수거집가중건출녀성분저기군,시연구녀성분저기군해부적유효방법;대항제기삼유모형진행양화분석,가평고분저공능,유조우리해분저공능장애성질병발병궤제,위림상진단、치료방법선택급료효평개제공삼고표준.
Objective To analyse anatomical characteristics of the pelvic floor in young nulliparous volunteers based on three-dimensional MRI.Methods Thin-slice MRI was performed in 25 young nulliparous volunteers in Southern Medical University,MRI were imported into Mimics 10.01 for 3D reconstruction.Using 3D models we measured follow indicators:the levator ani muscle volume (LVOL),levator plate angle (LPA),levator hiatus width (LH-W) and length (LH-L),distance between symphysis and levator sling muscle(LSG).Results (1)25 cases of pelvic three-dimensional models was successfully constructed,including the pelvis,pelvic organs and the pelvic floor muscles (including the ischial coccyx muscle,levator ani muscle and its various components,perineal muscles),the models could be able to clearly reflect the level of the pelvic floor muscles ; (2) 25 cases of levator ani muscle measurement results:LVOL:(34-±6) cm3,LPA:(43 ±4)°,LH-W:(33 ±4) mm,LH-L:(54 ±5) mm,left LSG:(18.8-± 2.5) mm,right LSG:(18.3 ± 2.5) mm.Conclusions It is an effective way to use the computer to reconstruct the 3D model of female pelvic floor muscles using MRI data set.The quantitative analysis of levator ani muscle three-dimensional model can be assessed pelvic floor function,which is of great value in clinical practice.It is helpful to understand the pelvic floor disorders pathogenesis,clinical diagnosis,treatment options and treatment evaluation to provide reference standards.
