目的总结分析大动脉炎合并皮肤黏膜损害的临床特点.方法回顾性分析我院2000-2012年住院的大动脉炎患者的临床资料；计量资料采用配对t检验.结果本研究共纳入487例大动脉炎患者,其中合并皮肤黏膜损害者38例(7.8％),女性31例,男性7例,平均年龄(26±11)岁.31例发生皮肤黏膜损害时病情活动,其中3例以皮肤黏膜损害起病.大动脉炎临床分型以广泛型(20例,53％)、头臂动脉型(10例,26％)多见.皮肤黏膜损害以口腔溃疡(19例,50％)、结节性红斑(9例,24％)和皮下结节(6例,16％)多见.皮肤病理提示血管炎和肉芽肿.经过大剂量糖皮质激素联合免疫抑制剂治疗,皮肤黏膜损害痊愈21例(55％),显效7例(18.4％),好转4例(11％),不详6例.结论大动脉炎除了大动脉受累,亦可出现多种皮肤黏膜病变,与大动脉炎病情活动密切相关,对糖皮质激素联合免疫抑制剂治疗的应答良好.
목적 총결분석대동맥염합병피부점막손해적림상특점.방법 회고성분석아원2000-2012년주원적대동맥염환자적림상자료；계량자료채용배대t검험.결과 본연구공납입487례대동맥염환자,기중합병피부점막손해자38례(7.8％),녀성31례,남성7례,평균년령(26±11)세.31례발생피부점막손해시병정활동,기중3례이피부점막손해기병.대동맥염림상분형이엄범형(20례,53％)、두비동맥형(10례,26％)다견.피부점막손해이구강궤양(19례,50％)、결절성홍반(9례,24％)화피하결절(6례,16％)다견.피부병리제시혈관염화육아종.경과대제량당피질격소연합면역억제제치료,피부점막손해전유21례(55％),현효7례(18.4％),호전4례(11％),불상6례.결론 대동맥염제료대동맥수루,역가출현다충피부점막병변,여대동맥염병정활동밀절상관,대당피질격소연합면역억제제치료적응답량호.
Objective To investigate the clinical features of cutaneous and mucosal manifestations in patients with Takayasu's arteritis (TA).Methods The clinical data of TA patients who were admitted into Peking Union Medical College Hospital from 2000 to 2012 were retrospectively reviewed.Resnlts Four hundred and eighty-seven TA patients were enrolled.Cutaneous and mucosal lesions were documented in 38 patients (31 female and 7 male).The mean age was (26±11) years old.The cutaneous and mucosal lesions occurred along with active disease in 31 patients,and before the onset of TA in 3 patients.The most common sub-types of TA were extensive type (53％) and head-arm-artery type (26％).Oral ulcer (50％),erythema nodosum (24％) and subcutaneous nodule (16％) were the most prevalent cutaneous and mucosa lesions.Biopsy specimens showed vasculitis and granulomatosis.Most patients (87％) were treated with corticosteroids and immuno-suppressive agents and improved clinically.Conclusion The cutaneous and mucosal lesions of TA are not uncommon,which suggest active disease in TA.The prognosis of cutaneous and mucosal lesions is generally good with aggressive treatment.
