目的 观察高压氧联合静脉滴注前列地尔治疗糖尿病足的临床疗效.方法 经患者知情同意后,将90例糖尿病足患者采用数字表法随机分为研究组、高压氧组和前列地尔组,每组30例.3组均予以糖尿病饮食、降血糖药物、营养神经、局部创面处理等常规治疗.研究组在此基础上,应用前列地尔20 μg加入生理盐水100 ml静脉滴注,1次/d,同时采用多人高压氧舱治疗,压力为0.20 MPa,1次/d,10次为1个疗程;高压氧组在常规治疗的基础上,采用高压氧治疗,治疗方法同研究组;前列地尔组在常规治疗的基础上使用前列地尔20μg加入生理盐水100 ml静脉滴注,1次/d.治疗4个疗程后进行临床疗效评定.结果 研究组、高压氧组和前列地尔组总有效率分别为93.3% (28/30)、76.7%(23/30)、70.0%(21/30),前者与后两者比较差异均有统计学意义(P<0.05).结论 高压氧联合前列地尔治疗糖尿病足有利于下肢动脉血供、氧供,改善微循环,提高溃疡愈合率,是治疗糖尿病足的有效方案.

Objective To study the clinical effect of hyperbaric oxygen (HBO) combined with liposomal alprostadil (Lipo-PGE1) in the treatment of diabetic foot.Methods Ninety patients with diabetic foot were randomly divided into the study group (HBO + liposomal alprostadil),the HBO group and the liposomal alprostadil group,each consisting of 30 cases.The patients of the 3 groups all received routine treatment,such as diabetic diet,hypoglycemic agent,trophic nerve and wound surface management.In addition to the routine treatment mentioned above,the patients in the study group received 20 μg intravenous drip diluted with 100 ml saline solution,once a day.At the same time,they had HBO therapy at a pressure of 0.2 MPa,once a day,10 sessions a treatment course.In addition to the same routine treatment,the HBO group received HBO therapy also at a pressure of 0.2 MPa,once a day,10 sessions a treatment course.With the same routine treatment mentioned above,the liposomal alprostadil group only received 20 μg intravenous drip diluted with 100 ml saline solution,once a day.After 4 treatment courses,therapeutic effects on the patients of the 3 groups were evaluated respectively.Results Following treatment,total effective rates for the study group,the HBO group and the liposomal alprostadil group were 93.3%,76.7% and 70.0% respectively.Significant differences in therapeutic effects could be noted,when the former was compared with the latter two groups (P < 0.05).Conclusions In the treatment of diabetic foot,HBO coupled with liposomal alprostadil could improve blood and oxygen supplies to the lower limbs,enhance microcirculation,increase healing rate of ulcers,and it was surely an effective treatment strategy for the treatment of diabetic foot.
