目的 对分离自新疆的泛耐药鲍曼不动杆菌中β-内酰胺酶基因的存在和变异情况进行分析.方法 收集2012年1至6月中国人民解放军乌鲁木齐总医院住院患者样本中分离的泛耐药鲍曼不动杆菌共20株,用聚合酶链反应(PCR)的方法分析8种A类β-内酰胺酶基因,4种β类β-内酰胺酶基因,2种C类β-内酰胺酶基因,5种D类β-内酰胺酶基因,并检测常用抗菌药物对菌株的最小抑菌浓度.结果 本组20株泛耐药鲍曼不动杆菌均同时携带了blaTEM、blaADC、blaOXA-2群、blaOXA-23群、blaOXA-51群等5种β-内酰胺酶基因,其中一株blaADC基因经测序后证明为blaADC新的变异型.20株菌株对头孢类、氨基糖苷类、氟喹诺酮类和碳青霉烯类抗菌药物均耐药,仅50%的菌株对阿米卡星敏感.结论 blaOXA-23和blaOXA-51是导致本组菌株对头孢类药物耐药的主要原因,blaADC检出新的变异型.

Objective To investigate the distribution and variation of β-lactamase genes in pandrug resistant Acinetobacter baumannii strains isolated from Xinjiang Uyghur Autonomous Region.Methods Twenty strains of pan-drug resistant Acinetobacter baumannii were collected from Urumqi General Hospital of People' s Liberation Army during January 2012 and June 2012.Eight class A β-lactamase genes,4 class B β-lactamase genes,2 class C β-lactamase genes,5 class D β-lactamase genes were analyzed by polymerase chain reaction (PCR).The minimum inhibitory concentrations of the commonly used antibiotics were determined.Results β-lactamase genes blaTEM,blaADC,blaOXA-2 group,blaOXA-23 group and blaOXA-51 group were positive in all 20 strains of Acinetobacter baumannii.By DNA sequencing,blaADC in strain No.18 was confirmed as a novel variant.The strains were resistant to cephalosporins,aminoglycosides,fluoroquinolones and carbapenems,and only 50% of strains were sensitive to amikacin.Conclusion In this group of Acinetobacter baumannii,β-lactamase genes blaOXA-23 and blaOXA-51groups are the leading causes of the resistance to cephalosporins,and a novel variant has been discovered in blaADC.
