目的 观察低碳水化合物肠内营养对AECOPD患者血气分析指标以及血清肿瘤坏死因子(TNF-α)、白介素-6(IL-6)、C反应蛋白(CRP)、人髓样分化因子(MyD88)的影响.方法 通过鼻饲管肠内给予AECOPD患者低碳水化合物肠内营养制剂,对照组采用正常营养匀浆制品.检测血气分析、IL-6、TNF-α、MyD88、CRP等指标.结果 治疗第7d与对照组比较,治疗组PaO2高于对照组,PaCO2、IL-6、TNF-α、MyD88、CRP水平均低于对照组,差异有统计学意义(P<0.05).结论 短期低碳水化合物肠内营养可改善AECOPD患者通气水平,并可以降低AECOPD患者炎症水平.

목적 관찰저탄수화합물장내영양대AECOPD환자혈기분석지표이급혈청종류배사인자(TNF-α)、백개소-6(IL-6)、C반응단백(CRP)、인수양분화인자(MyD88)적영향.방법 통과비사관장내급여AECOPD환자저탄수화합물장내영양제제,대조조채용정상영양균장제품.검측혈기분석、IL-6、TNF-α、MyD88、CRP등지표.결과 치료제7d여대조조비교,치료조PaO2고우대조조,PaCO2、IL-6、TNF-α、MyD88、CRP수평균저우대조조,차이유통계학의의(P<0.05).결론 단기저탄수화합물장내영양가개선AECOPD환자통기수평,병가이강저AECOPD환자염증수평.
Objective To observe effect of enteral nutrition support with low carbohydrate on blood gas analysis indexes and serum inflammatory factors of patients with AECOPD.Methods Patients in test group and control group were fed by nasal tube for 7 days,with low carbohydrate for test group and normal homogenate for control group.Detected and analyzed levels of blood gas analysis indexes,IL-6,TNF-α,MyD88 and CRP of patients in two groups.Results 7 days after treatment,the level of PaO2 in test group was higher than that in control group,the levels of PaCO2,IL-6,TNF-α,MyD88,CRP in test group were lower than those in control group,with statistically significant differences between two groups (P < 0.05).Conclusion Enteral nutrition support with low carbohydrate can improve ventilation function and reduce inflammation levels of patients with AECOPD.
