目的 探讨大鼠窒息性心脏停搏复苏(CA-CPR)后心肌微小RNA (miRNA)表达的动态变化.方法 清洁级成年雄性SD大鼠64只,体重250 ~ 330 g,采用随机数字表法,将其分为2组(n=32),对照组(C组)和CA-CPR组.采用窒息法建立CA-CPR模型.CA-CPR组于自主循环恢复后3、6、12和24 h(T1-4)时,C组于相应时点取血样,测定血清CK-MB和cTnI浓度,相应时点取心尖部心肌组织,采用Real-Time PCR法检测心肌组织miRNA.以相对于C组心肌miRNA 2倍强度的变化作为CA-CPR组miRNA表达上调或下调的标准.记录大鼠均发生表达变化的miRNA.结果 与C组比较,CA-CPR组各时点血清CK-MB和cTnI浓度升高(P<0.01);CA-CPR组血清CK-MB浓度T3时达峰值,血清cTnI浓度持续升高(P<0.05或P<0.01).自主循环恢复后持续性变化的miRNA有8个,分别为miRNA-1、miRNA-21、miRNA126、miRNA-133a、miRNA-206、miRNA-210、miRNA-320及miRNA-499.miRNA-1各时点表达均上调,T2时最高;miRNA-21各时点表达均下调,T2时最低;miRNA126、miRNA-133a、miRNA-206、miRNA-210和miRNA-499 T1、T2时表达下调,T2时最低,T3、T4时表达上调;miRNA-320T1 ~T3时表达上调,T2时最高,T4时表达下调(P<0.05或0.01).结论 心脏停搏大鼠复苏后有8个miRNA表达发生变化,且变化方式(上调/下调)、趋势不同,但表达变化方式改变的时点相似.

Objective To investigate the dynamic changes in myocardial microRNA (miRNA) expression after cardiopulmonary resuscitation in a rat model of asphyxial cardiac arrest.Methods Sixty-four adult male Sprague-Dawley rats,weighing 250-330 g,were randomly divided into 2 groups (n =32 each):control group (group C) and cardiopulmonary resuscitation after cardiac arrest group (group CA-CPR).The rats were anesthetized with intraperitoneal chloral hydrate 7 ml/kg,tracheostomized and mechanically ventilated.The model of cardiac arrest was established by clamping the endotracheal tube at end expiration and cardiopulmonary resuscitation was performed.At 3,6,12 and 24 h after recovery of spontaneous circulation (T1-4),blood samples were collected for determination of serum creatine-kinase isoenzyme MB (CK-MB) and cardiac troponin I (cTnI) concentrations and myocardial specimens were obtained from the apex for detection of the expression of miRNAs in myocardial tissues (by using RT-PCR).Results Compared with group C,serum CK-MB and cTnI concentrations were significantly increased at each time point in group CA-CPR (P < 0.01).The serum CK-MB concentration reached the peak at T3,and the serum cTnI concentrations continued to increase in group CA-CPR (P < 0.05 or 0.01).There were 8 miRNAs which changed continuously after recovery of spontaneous circulation,and there were miRNA-1,miRNA-21,miRNA126,miRNA-133a,miRNA-206,miRNA-210,miRNA-320 and miRNA-499.The expression of miR-NA-1 was up-regulated at each time point and peaked at T2.The expression of miRNA-21 was down-regulated at each time point and reached the lowest level at T2.The expression of miRNA126,miRNA-133a,miRNA-206,miRNA-210 and miRNA-499 was down-regulated at T1 and T2,was up-regulated at T3 and T4,and reached the lowest level at T2.The expression of miRNA-320 was up-regulated at T1-T3,was down-regulated at T4,and peaked at T2 (P < 0.05 or 0.01).Conclusion Eight miRNAs exhibit changes in expression after cardiopulmonary resuscitation in a rat model of asphyxial cardiac arrest and the way and tendency that miRNAs have changed are different,but the time point at which the way that miRNAs have changed is similar.
