目的 观察葛兰心宁软胶囊对非ST段抬高型急性冠状动脉综合征(NSTE-ACS)患者经皮冠状动脉介入(PCI)术后内皮功能的影响.方法 将入院行PCI术的100例NSTE-ACS患者完全随机分为对照组和观察组,各50例.对照组给予常规治疗,观察组在常规治疗基础上加服葛兰心宁软胶囊4粒(0.58 g/粒),3次/d,2组疗程均1个月.于PCI术前空腹12 h和术后1个月抽血检测患者血脂、NO和内皮素1水平;PCI术前和术后1个月采用高分辨率彩色多普勒超声仪测定患者肱动脉内皮依赖性血流介导的血管舒张功能(FMD).结果 与PCI术前相比,观察组患者术后1个月血脂水明显改善,差异有统计学意义(P<0.05);与对照组相比,用药1个月后观察组TG、TC、LDL-C明显降低,差异有统计学意义(P<0.05);2组患者PCI术后1个月内皮素1较术前降低[对照组:(66±10) ng/L比(74±18) ng/L,观察组:(61±15) ng/L比(75±24) ng/L,P<0.05],NO较术前增加[对照组:(101±8) μmoL/L比(79±8)μmol/L,观察组:(131±7) μmol/L比(79±11) μmol/L,P<0.05],其中观察组较对照组升高更为明显(P<0.05);对照组与观察组患者PCI术后1个月的FMD较术前升高[(8.6±1.4)%比(5.8±2.2)%,(9.0±1.3)%比(5.8±2.1)%,P <0.05],观察组FMD升高更明显(P<0.05).结论 NSTE-ACS患者PCI术后在常规治疗基础上加用葛兰心宁软胶囊能进一步改善内皮功能和血脂水平.

Objective To investigate the effects of Gelanxinning soft capsules on endothelial function after percutaneous coronary intervention (PCI) in non-ST-segment elevation acute coronary syndrome (NSTE-ACS) patients.Methods One hundred NSTE-ACS patients who would recept PCI were randomly divided into control group (n =50) and observation group (n =50).Patients in the control group were given conventional treatment,while patients in the observation group were treated with 4 Gelanxinning soft capsules three times a day for one month besides conventional treatment.The lipid and NO levels in serum and endothelin-1 level in plasma of all patients were detected before PCI and 1 month after PCI.Endothelium-dependent flow-mediated dilation (FMD) in all patients were measured on the day of admission and one month after the day of onset of PCI.Results Compared with the lipid level before PCI,the levels of lipid in both groups were lower one month after PCI (P < 0.05).The levels of TG,TC and LDL levels in the observation group were much lower than in the control group(P < 0.05).The level of endothelin-1 in both groups was lower[(66 ± 10) ng/L vs (74 ± 18) ng/L,(61 ± 15) ng/L vs (75 ± 24) ng/L,P <0.05] and NO was higher[(101 ± 8) μmo]/L vs (79 ±8)μmol/L,(131 ±7) μmol/L vs (79 ± 11) μmol/L,P<0.05] after one month,and the observation group was more significantly improved than the control group (P <0.05).The level of FMD one month later in both groups was higher than that on the day of admission [(8.6 ±1.4) % vs (5.8 ± 2.2) %,(9.0 ± 1.3) % vs (5.8 ± 2.1) % (P < 0.05),while the level in the observation group was much higher than that in control group (P < 0.05).Conclusion Gelanxinning soft capsules can further improve endothelial function and the lipid level after PCI in NSTE-ACS patients.
