目的探讨腹腔镜胆囊切除术用于临床治疗慢性胆囊炎合并胆囊结石的应用价值.方法回顾性分析201 1年6月至2013年9月在我院行手术治疗的245例慢性胆囊炎合并胆囊结石患者的临床资料,按照不同手术治疗方式分为实验组(153例,行腹腔镜胆囊切除术治疗)和对照组(92例,行传统开腹手术治疗),对两组患者术中、术后情况进行综合比较.结果两组患者术中出血量、术后首次肛门排气时间及住院时间等手术指标比较,差异均有统计学意义(P＜0.05);实验组患者术后切口感染、胆汁漏、胆管损伤发生率明显低于对照组(P＜ 0.05).结论腹腔镜胆囊切除术治疗慢性胆囊炎合并胆囊结石效果显著,达到和开腹手术同样的探查及治疗效果,并且具有恢复快、并发症少等优点,值得临床推广应用.
목적 탐토복강경담낭절제술용우림상치료만성담낭염합병담낭결석적응용개치.방법 회고성분석201 1년6월지2013년9월재아원행수술치료적245례만성담낭염합병담낭결석환자적림상자료,안조불동수술치료방식분위실험조(153례,행복강경담낭절제술치료)화대조조(92례,행전통개복수술치료),대량조환자술중、술후정황진행종합비교.결과 량조환자술중출혈량、술후수차항문배기시간급주원시간등수술지표비교,차이균유통계학의의(P＜0.05);실험조환자술후절구감염、담즙루、담관손상발생솔명현저우대조조(P＜ 0.05).결론 복강경담낭절제술치료만성담낭염합병담낭결석효과현저,체도화개복수술동양적탐사급치료효과,병차구유회복쾌、병발증소등우점,치득림상추엄응용.
Objective To investigate the value of laparoscopic cholecystectomy for chronic cholecystitis complicated with gallstones.Methods The clinical data of 245 patients with chronic cholecystitis complicated with gallstones surgically treated at our hospital from June,2011 to September,2013 were retrospectively analyzed.The patients were divided into an LC group (153 cases,received laparoscopic cholecystectomy) and an OC group (92 cases,received open cholecystectomy).The intraoperative and postoperative conditions of both groups were compared.Results There were statistical differences in intraoperative bleeding volume,postoperative first anus exhausting time,and hospitalization time etc.between the two groups(P ＜ 0.05).The incidences of incision infection,bile leakage,and bile duct injury were lower in LC group than in OC group(P ＜ 0.05).Conclusions Laparoscopic cholecystectomy for chronic cholecystitis complicated with gallstones is effective,has the same exploration and treatment effect as conventional open cholecystectomy,can recover the patients quickly and reduce complications,and is worth being clinically generalized.
