目的探讨腹腔镜手术治疗胃肠道间质瘤(gastrointestinal stromal tumor,GIST)的临床应用价值及安全性.方法回顾分析华中科技大学同济医学院附属协和医院2005年1月至2010年9月间经腹腔镜手术治疗的35例GIST患者的临床资料.结果本组35例GIST患者中包含胃GIST 27例,小肠GIST 5例,直肠GIST 3例.肿瘤中位直径3 cm(0.8 ～ 16.0 cm).除1例因肿瘤较大而中转开腹外其余均顺利完成手术,平均手术时间(104±32) min,术中出血量(25 ±30) ml,排气时间(36±15)h,术后住院(8.4±1.8)d,无手术并发症及死亡病例.中位随访44个月(22～78个月),均未见肿瘤复发或转移.结论对于GIST,特别是直径＜5 cm者,行腹腔镜手术治疗安全可靠,术式选择应依据肿瘤大小与位置进行个体化治疗.

Objective To evaluate the clinical value and safety of laparoscopic resection for gastrointestinal stromal tumors (GISTs).Methods A retrospective analysis was made on thirty-five patients who underwent laparoscopic resection for GISTs in Wuhan Union Hospital from January 2005 to September 2010.Results There were 27 gastric GIST cases,5 intestinal GIST cases and 3 rectal GIST cases.The median tumor size was 3 cm (range 0.8-16.0 cm).All operations were successfully accomplished within (104 ± 32) min except one who was shifted to open surgery for large size of the tumor.The intraoperative blood loss was (25 ± 30) ml and exhaust time was (36 ± 15) h.The postoperative hospital stay was (8.4 ± 1.8) days.There was no major morbidities nor mortality.During the follow-up time from 22 to 78 months (median,44 months),there were no recurrences or metastasis.Conclusions The low morbidity rate and the long-term disease-free survival indicates that laparoscopic resection for GISTs with diameter of tumor size ＜ 5 cm is safe and feasible.
