目的探讨经口入路腔镜甲状腺切除手术的方法与结果.方法回顾性分析2011年11月至2013年4月收治并经口入路腔镜甲状腺切除手术37例患者的临床资料.结果 37例均顺利完成经口入路腔镜下甲状腺手术,平均手术时间109.5 min;术中出血量15～60 ml;术后平均住院天数3.5d.3例术后发生颈前部皮肤软组织感染,1例鼻塞,暂时性嗅觉障碍,给予对症处理均康复.所有病例均无声音嘶哑、继发性出血或血肿等并发症,术后随访0.5 ～15个月,患者均无明显不适感,手术美容效果佳.结论经口入路腔镜甲状腺切除手术创伤小、恢复快、并发症少且术后美容效果好,是一种安全、有效、可行的术式.

Objective To evaluate the outcomes of totally transoral video-assisted thyroidectomy.Methods Clinical data of 37 cases of totally transoral video-assisted thyroidectomy from November 2011 to April 2013 at Zhongshan Hospital,Xiamen University were analyzed retrospectively.Results All cases underwent totally transoral video-assisted thyroidectomy successfully with a mean operation time of 109.5 min,blood loss of 15-60 ml,the mean postoperative hospital-stay was 3.5 days.The postoperative complications included infection in three cases,nasal congestion in one case,transient dysosphresia in one case.All were subject to symptomatic management and were recovered.There were no postoperative hoarse voice,secondary bleeding nor hematoma.With a follow-up of 0.5 to 15 months,no adverse events were found in all cases,patients were satisfied with the cosmetic result of the operation.Conclusions Totally transoral video-assisted thyroidectomy (TOVAT) is a safe and effective technique with the advantages of minor injury,quicker recovery,fewer complications and good cosmetic effect.
