目的 探讨三叉神经痛及面肌痉挛微血管减压术前MRI检查对手术的指导意义.方法 分析江门市中心医院神经外科自2008年1月至2012年12月收治的57例三叉神经痛患者与18例面肌痉挛患者的临床资料,所有患者术前均行三维时间飞跃法磁共振血管成像(3D-TOF-MRA)检查,其中6例同时行三维稳态进动快速成像序列(3D-FIESTA) MRI检查,分析所有患者微血管减压术前3D-TOF-MRA和3D-FIESTA检查的临床意义. 结果 三叉神经痛及面肌痉挛患者血管神经关系判断中,术前MRI检查与术中所见结果差异无统计学意义(P>0.05).将神经与血管关系为接触者归类为阳性.MRI检查阳性率与术中所见的阳性率亦较接近. 结论 术前3D-TOF-MRA及3D-FIESTA MRI检查能够识别三叉神经痛及面肌痉挛患者患侧相应神经和邻近血管的关系,有助于术前制定手术方案,提高手术疗效.

Objective To investigate the role of preoperative three dimensional magnetic resonance imaging in predicting the clinical outcomes of microvascular decompression for the treatment of trigeminal neuralgia and hemifacial spasm.Methods Fifty-seven patients with trigeminal neuralgia and 18 with hemifacial spasm,admitted to our hospital from January 2008 to December 2012,were included in our study.All the patients were performed 3D time-of-flight magnetic resonance angiography (3D-TOF-MRA) prior to microvascular decompression,and 6 of them also performed 3D fast imaging employing steady-state acquisition (3D-FIESTA) MRI.In all patients,the presence of vascular contact,offenders were determined by 3D-TOF-MRA and 3D-FIESTA MRI,and the preoperative findings were compared with the surgical findings and clinical outcomes.Results Between the symptomatic sides and asymptomatic sides,the difference of vascular contact manifested by MRI was statistically significant (P<0.05).The coincidence of MR imaging and sight during the operation was similar.Conclusion Preoperative 3D-TOF-MRA and 3D-FIESTA can identify the relationship between the nerve and adjacent vessels in patients with trigeminal neuralgia or hemifacial spasm,which assists in preoperative planning and improve the operation effect.
