目的 探讨微型锁定加压钢板(locking compression plate,LCP)治疗复杂性掌、指骨骨折的疗效.方法 2009年6月至2011年8月,对30例复杂性掌、指骨骨折应用微型LCP行复位内固定治疗,辅以早期功能锻炼及正确的康复指导.结果 术后30例伤口全部Ⅰ期愈合,石膏外固定时间为2~3周(平均2.1周).术后随访5~8个月,平均7个月.1例掌骨骨折出现骨不连,经植骨内固定后愈合;余29例骨折全部愈合,时间为6~12周,平均10周.按手指总主动活动度(TAM)功能评定:优22例,良4例,中3例,差1例;优良率达86.7%.结论 微型LCP内固定治疗复杂性掌指骨骨折,固定可靠,可早期进行功能锻炼,有利于手功能的恢复.

목적 탐토미형쇄정가압강판(locking compression plate,LCP)치료복잡성장、지골골절적료효.방법 2009년6월지2011년8월,대30례복잡성장、지골골절응용미형LCP행복위내고정치료,보이조기공능단련급정학적강복지도.결과 술후30례상구전부Ⅰ기유합,석고외고정시간위2~3주(평균2.1주).술후수방5~8개월,평균7개월.1례장골골절출현골불련,경식골내고정후유합;여29례골절전부유합,시간위6~12주,평균10주.안수지총주동활동도(TAM)공능평정:우22례,량4례,중3례,차1례;우량솔체86.7%.결론 미형LCP내고정치료복잡성장지골골절,고정가고,가조기진행공능단련,유리우수공능적회복.
Objective To investigate the treatment outcomes of internal fixation of complex metacarpal and phalangeal fractures using mini locking compression plates (mini-LCP).Methods From June 2009 to August 2011,30 cases of complex metacarpal and phalangeal fractures were treated with mini-LCP followed by early functional training under proper rehabilitation guidance.Results Primary wound healing was achieved in all 30 cases.The length for immobilization with a plaster cast was 2 to 3 weeks (average 2.1 weeks).Postoperative follow-up period ranged from 5 to 8 months.One case had metacarpal nonunion which was healed by bone graft and internal fixation.The rest 29 cases had fracture union 6 to 12 weeks (average 10 weeks) after the surgery.Functional evaluation based on total active motion (TAM) scale yielded an excellent and good rate of 86.7 %.There were 22 cases with excellent results,4 cases with good results,3 cases with fair results and 1 case with poor result.Conclusion Internal fixation with mini-LCP in the treatment of complex metacarpal and phalangeal fractures can achieve stable fixation that allows early functional rehabilitation and facilitates hand function restoration.
