目的 探讨桡动脉茎突返支骨瓣移植加桡骨茎突切除治疗腕舟骨骨不连的手术疗效.方法 采用腕舟骨切开复位,桡动脉茎突返支骨瓣移植加桡骨茎突切除,螺丝钉内固定治疗腕舟骨骨不连12例.结果 术后10例获得随访,时间为3~ 30个月;2例失访.术后8~ 16周(平均11.8周)X线片显示骨折线均已消失,原坏死囊性变区域已有骨小梁通过.腕关节功能优良率达90%.结论 桡动脉茎突返支骨瓣移植加桡骨茎突切除治疗腕舟骨骨不连是一种有效可靠的方法,促进骨不连愈合效果好,减少骨关节炎的发生.
目的 探討橈動脈莖突返支骨瓣移植加橈骨莖突切除治療腕舟骨骨不連的手術療效.方法 採用腕舟骨切開複位,橈動脈莖突返支骨瓣移植加橈骨莖突切除,螺絲釘內固定治療腕舟骨骨不連12例.結果 術後10例穫得隨訪,時間為3~ 30箇月;2例失訪.術後8~ 16週(平均11.8週)X線片顯示骨摺線均已消失,原壞死囊性變區域已有骨小樑通過.腕關節功能優良率達90%.結論 橈動脈莖突返支骨瓣移植加橈骨莖突切除治療腕舟骨骨不連是一種有效可靠的方法,促進骨不連愈閤效果好,減少骨關節炎的髮生.
목적 탐토뇨동맥경돌반지골판이식가뇨골경돌절제치료완주골골불련적수술료효.방법 채용완주골절개복위,뇨동맥경돌반지골판이식가뇨골경돌절제,라사정내고정치료완주골골불련12례.결과 술후10례획득수방,시간위3~ 30개월;2례실방.술후8~ 16주(평균11.8주)X선편현시골절선균이소실,원배사낭성변구역이유골소량통과.완관절공능우량솔체90%.결론 뇨동맥경돌반지골판이식가뇨골경돌절제치료완주골골불련시일충유효가고적방법,촉진골불련유합효과호,감소골관절염적발생.
Objective To investigate the effect of vascularized bone graft pedicled on the styloid recurrent branch of the radial artery and excision of radial styloid for management of scaphoid nonunion.Methods Twelve cases of scaphoid nonunion were treated with open reduction and internal fixation of the scaphoid.At the same time the radial styloid was excised and a bone flap pedicled on the styloid recurrent branch of the radial artery was grafted.The range of follow-up is from 3 to 30 months.Results Postoperatively 10 cases were follow-up for 3 to 30 months,while 2 cases were lost to follow-up.Radiographic evidence of fracture union was seen between 8 and 16 weeks postoperatively (mean,11.8 weeks).The necrotic and cystic areas were replaced by bone trabeculae.Good wrist function was achieved in 90% of the cases.Conclusion Vascularized bone graft pedicled on the styloid recurrent branch of the radial artery combined with radial styloid excision for the management of scaphoid nonunion is effective and reliable.It can cure the nonunion and reduce the occurrence of osteoarthritis.
