目的探讨腕关节镜下舟骨骨不连的治疗方法及临床疗效.方法 2011年1月至2012年5月,对6例舟骨腰部骨不连的患者采用腕关节镜辅助下,去除硬化骨、克氏针固定术加自体骨植骨治疗.术后定期拍摄X线片,观察骨折愈合情况.采用改良Mayo腕关节评分评估患者腕关节功能.结果所有骨折均获得愈合,平均愈合时间13.7周.6例患者均获得随访,平均随访时间7.5个月.改良Mayo腕关节评分:优2例,良3例,可1例.结论对舟骨骨不连的患者,在掌握适应证的前提下,采用腕关节镜下去除硬化骨、克氏针内固定及自体骨植骨,能取得较理想的临床效果.

Objective To analyze the preliminary clinical results of treating scaphoid nonunion with arthroscope assisted internal fixation and autologous bone grafting.Methods From January 2011 to May 2012,a consecutive series of 6 patients with nonunion of fractures of the scaphoid waist were treated with arthroscope assisted debridement of the sclerotic bone,percutaneous K-wire fixation and autologous bone graft.Postoperative evaluations included measurement of clinical (grip strength and range of motion),radiographic,and functional (modified Mayo wrist score) parameters.Results All the scaphoid nonunions were healed with an average healing time of 13.7 weeks.All 6 cases were follow-up for an average of 7.5 months.The function was rated as excellent in 2 cases,good in 3 cases and fair in 1 case according to the modified Mayo wrist score.Conclusion When properly indicated,arthroscope assisted debridement of sclerotic bone,percutaneous K-wire fixation along with autologous bone graft is a reliable and minimally invasive method to treat scaphoid nonunion.
